Notification:
On 10/19/2016 at approximately 0830 hrs. I was dispatched to E St. between 11th and 12 th St for an injury traffic collision. I was requested by California Department of Forestry and Fire protection (CDF) to respond code three to their location. I then responded code three with my overhead lights and siren activated. I arrived on scene and saw that Riggs Ambulance service and CDF were already on scene administering first aid to a male in the roadway
All times, speeds and measurements are approximate. All measurements were made using a Livingston Police Department roll-a-tape from Unit 5.

These pieces of plastic are most likely from the damage caused to V2 by V1. There was a black shoe and a black backpack located near the middle of E St. These items were worn by P1 prior to the collision. Additionally there was a large pool of blood where P1 was lying down at when I arrived on scene. I took several photographs of the scene and miscellaneous items. These photographs were downloaded onto a CD and booked into the LPD evidence room. For further refer to the Factual Diagram Additionally the scene was recorded using my department issued body camera.

Hit-and-Run:
On the day of the collision Witness John McDaniels (W1) reported that a Red Honda Accord (CA: 3PPT728) was driving westbound on E St. immediately after the collision occurred.
W1 advised there was only one occupant of V2, the driver P2. W1 described P2 as a white female adult in her 30’s with brown hair and wearing a red shirt.

Other factual information:
Dispatch advised that the license plate obtained by W1 came back registered to a Michael Phillips at 1338 Pacific St. #334, Stockton CA
