In reference to report 15-05766, I, Investigator Nicole Anderson, report the following, On Thursday, December 10, 2015, I spoke with Patient Chandra King in regards to money missing from her wallet. King started she asked her son to hold her wallet while she went to the restroom located on the fourth floor inside the Center for Outpatient Health. King stated after she departed the restroom, she proceeded to her doctor appointment, which was located on the third floor. While on the elevator she asked her son for her wallet and he stated, he left the wallet is on top of the water fountain next to the restroom on the fourth floor. King stated she proceeded back to the fourth floor to retrieve her wallet. Upon arriving to the water fountain,
Kim Tutt, is a survivor of a very serious malpractice case. At the time of the event, she was only 32 years old and completely healthy with two 12-year-old sons. After going to the dentist for x-rays, she was told that she had jaw cancer and was given only 3-6 months to live. But, she could add another 3 months to her life, if they removed the left side of her chin all the way up to her ear and use her fibula, from her leg, as a replacement. Out of desperation she went ahead with the aggressive surgery. About a month after the completion of the surgery, she was contacted and notified that the specimen which tested positive for cancer was a result of cross-contamination in the lab and she actually did not have cancer. The job she is applied for could potentially have a lot of interaction with clients in the rehab with a low pay of $10 an hour. In her interview, she informs the interviewer that she in on a “maintenance drug” called Suboxone. The drug is helpful when it comes to getting people off heroin and staying sober. This is a new drug on the market and the rehab is not sure about this drug, therefore they choose not to use it. Spoke with Jade Bray, CHR therapist with regarding to Valerie treatment. This writer questioned Ms. Bray about her concerns and the mental health provider intervention for the patient due to the 3 suicidal ideation as a result of stresses in the patient's life. According to Ms. Bray, she expressed her concerns of the patient not following through with recommendations, mainly referring to Chrysalis for assistance with housing, the patient misuse of funds, and discontinuing her use of her addiction. Next week, according to the therapist, there will be a meeting-Multidimensional Therapy (MDT) meeting to address further intervention(s) for the patient. Ms. Floyd received information from Ms. Goods stating she’d put her mother out of her home because of her crack use. Brittney was not put out of the home and is currently residing with her sister. Brittney’s basic needs are being met by her sister and Ms. Goods appears to be a better and more responsible caretaker. Brittney has been in and out of the court system and Ms. Higgins has legal custody of the child at this time; Brittney is without a legal guardian since her mother was put out of the home. It’s unknown what date Ms. Higgins was put out but the information was reported on 08/11/15.

As the department manager of a large commercial nuclear generating plant and a member of the Plant Nuclear Safety Review Committee (PNSRC), Alison Turner’s action or inaction is quite critical. In the present case, the facility’s heat exchanger has a problem, which, according to Turner, may create significant problems. To be precise, two heat exchangers are operating at 95 percent due to a sand blockage on the outlet to the lake water side of the four heat exchangers. After an extensive analysis, engineers in the Mechanical Engineering and Nuclear Safety and Licensing Department concluded that the heat exchangers met the minimum cooling requirements set by technical specifications. The Nuclear Safety and Licensing Board also offered a Justification for Continued Operation (JCO) for submission to the Nuclear Regulatory Commission. However, Turner is still adamant because she feels other PNSRC members should not approve the JCO because they have not analyzed possible consequences in case one of the heat exchangers fails permanently.
