Case Study The case is about Roberta, a sixty-nine-year-old woman a voluntary client who sought out therapy for her depression. The Roberta's mother passed away a year ago, and her mother was the primary caregiver to her brother, Jonathan, but since her passing Roberta has the caregiver responsibility since her sister Geraldine lives further away from both Jonathan and Roberta. There is also the issue of inheritance because the mother’s house is being sold and split between all three, but Johnny might lose his benefits if he gets the money from the mother’s house being sold. Her brother is sixty-two is with bipolar disorder which disables him. Roberta and her brother do not get along and wish not to live with him. Roberta also found out her rent for her apartment is increasing. Roberta discusses her thoughts about dying to reunite with her mother, but no ideation of suicide. System theory helps look at the various system a client may be a part of and see how the systems influence a person’s behavior and thoughts. System theory evaluates a person in the environment. Roberta is with depression, but her system help contributes to her depression. Roberta’s family system is going through the death of a loved one and responsibility of care has been bestowed to Roberta with little help. Roberta’s housing system is increasing in rent causing money stress, and her not being able to be with her support system due to financial stress. Roberta is isolated from the systems that help with her mental health due to money restriction but is reluctant to move to less expensive housing because she would be leaving her community. Roberta is involved in the community by being on the Board of Directors of the City Library, and she says that it gives her pleasure in involvement. Shanell Bathersfield conducted a self-assessment. She initially determined that the systems theory was one that she was not comfortable with. Through the last few course weeks, she was able to gain an in-depth perspective on several theories that are impactful in the social work field. The coursework and utilizing Juana and Adelina as case studies, truly gave an exclusive insight on how theories are used to understand the client’s circumstances and improve their well-being.
Nursing care is a dynamic field of practice. The way it looks today is far out greater intense and very structured. It advances itself by the use of nursing theories and evidence based practice. Policies and procedures constantly change with the advancement of technology and science. While caring for the patient in the given case studies, a nurse involved utilizes practical knowledge, a culture care model and transpersonal caring relationship to attain a caring environment (Smith & Parker, 2015).
The aim of this study is to provide a detailed account of the nursing care for a patient who is experiencing a breakdown in health. One aspect of their care will be discussed in relation to the nursing process. The model used to provide an individualised programme of care will be discussed and critically analysed.

System perspective see’s human behaviors as the outcome of interactions within and among social systems of interrelated parts. These social systems of interrelated parts consist of people within the family, friends, school and work. System perspectives can be understood as the subsystem of larger systems, where each system has its unique identity but they are all interdependent. From a system perspective, behaviors are influenced from the past which are learnt from within the family and other social systems. Within a system perspective, there are smaller systems that a Social Worker may consider for a more holistic view. This is done by considering the role the physical and social environment has on a person’s behavior as well as protective and risk factors that influences positive and negative outcomes.
The systems Perspective sees human behavior as the outcome of interactions within and among systems with interrelated and interdependent parts; as defined by its boundaries. Changing one part of the system affects other parts and the whole system, with predictable patterns of behavior (Hutchinson, 2015). The family system has been a widely used theory of family interventions and assessment. The family systems theory is focused on the family dynamic, involving structures, roles, communication patterns, boundaries, and power relation (Rothbaum, 2004). As per OMNI, she was diagnosed with muscle spasm, pain over the low back and thoracic region. She is status post right carpal tunnel release on 02/26/16.
B.T. has a nursing diagnosis of ineffective airway clearance that requires nurse management with prescribed beta 2 adrenergic agonists, and teaching effective coughing and breathing techniques. The respiratory therapist will assist by performing nebulizer treatment and teaching the patient about home nebulizer. The nurse will emphasize on the importance of adhering to medication regimen and taking the right medication at the right time.

Patient explains that when she experiences pain, she engages in herself in self-talk by saying “S... you can do this.” When she starts loosing hope and thinking of giving up, she prays to Jesus and believes that he chooses what is best for her. Taking with herself and praying give the patient a strength to overcome the situations she is experiencing. The patient states that talking to self does not help her overcoming some situations such as excruciating pain. However, she eventually returns to her normal coping system after the challenging episodes of pain.
Systems theory is one aspect to include in all cases because it is general knowledge. The questions that the social worker asked helped create a plan, and an end goal for Mrs. Washburn. The individual’s needs, expectations and rewards that come from having the help from a social worker should always be included. Systems theory also creates a way for the individual to take charge, and accomplish their goals themselves. The social worker based their questions off of systems theory because it was the easiest way to get general information.
Patricia is an 84-year-old female who is diagnosed with metastatic colon cancer (C18.9). She also suffers from anemia. Patricia underwent a right hemicolectomy and a partial omentectomy, with noted bulky adenopathy at the base of the mesentery again, this proved to be moderately differentiated adenocarcinoma with mucinous features with venous, lymphatic and perineural invasion present. 22/32 lymph nodes were involved with cancer, margins were negative, pathologically this was a T3 N2b cancer. Patricia was treated with 5-FU and leucovorin but still experienced disease progression, with adverse side effects and found to have metastasis to the liver and bone which is KRAS and NRAS wild type. Mrs Banks was then taken back to the consulting room to be with her husband. From the health history and physical examination findings, a definite diagnosis was not obtained and it was explained that further investigation were needed to establish the cause of the pain and bleeding. Upon reviewing the differential diagnosis, the possibility of ectopic pregnancy was identified, this presents a major health problem for women of childbearing age. Without timely diagnosis and treatment ectopic pregnancy can be a life threatening situation (RCOG 2016).It is the leading cause of maternal death in the first trimester of pregnancy accounting for 10-15% of all pregnancy related deaths (National Institute for Health and clinical Excellence (NICE) 2015)
