On November 15, 2015 at approximately 0052 hrs., I was parked at O'Reilly Auto Parts, 200 S. Main St., Brookfield, MO 64628. While sitting at O'Reilly Auto Parts, I observed a black in color truck traveling southbound on S. Main St. I noticed the truck had its running lights running but failed to display lighted lamps on a motor vehicle. I activated my red and blue emergency lights noticing the driver crossing the white center line two times before the truck came to a complete stop.
The truck came to a stop in the 600 block of S. Main St. I made contact with the driver. I asked the driver how it was going. The driver advised "not very good." I asked the driver for his driver's license. The driver handed me his driver's license. I

I instructed Ellison to imagine a straight line from his toes about 20 feet out. Ellison advised he could see the line. I asked Ellison to place his left foot on that line and place his right foot in front of his left foot heel-to-toe. I demonstrated the instruction to Ellison. I advised Ellison to then take 9 heel-to-toe steps forward keeping the lead foot on the line, taking a series of small steps and then take 9 heel-to-toe steps back. I asked Ellison if he understood and he then began the test. Ellison couldn't maintain heel-to-toe steps taking 3 steps forward and 3 steps back. I instructed Ellison to perform the test taking 9 heel-to-toe steps forward and 9 heel-to-toe steps back that I performed and explained to him. Ellison failed to maintain heel-to-toe through out the steps 2,3,4,5,6,7,8,9. Ellison stepped off line on the 3rd step Ellison failed to take a series of smalls steps as instructed. Ellison failed to maintain heel-to- toe steps 1,2,3,4,5,6 and stopped at the 6th step turning around thinking the test was done. I advised Ellison to nine and he only counted to six. Ellison finished the test without maintaining heel-to-toe on step 7-8-9. When I asked Ellison if he was under the influence of an alcoholic beverage, Ellison advised yes. When asked if Ellison had used marijuana or any other drug, legal or illegal, in the last 72 hours, Ellison advised no.
I transported Ellison to Pershing Memorial Hospital. Ellison's blood was drawn at approximately 0154 hrs. and again at approximately 0234 hrs. by Lindsee R. Buckallew, RN. Once Ellison's blood was drawn, I transported Ellison back to the Brookfield Police Department for further processing.

Once on scene I observed the described black Ford F-150 pickup truck. The truck was positioned facing westbound and in the roadway on the unit block of East Laurel Street, where Laurel Street intersects with Main Street (State Route 924). On Thursday, July 14, 2016, at approximately 0847 hours, Patrolman David Stamets had a vehicle stopped on the 100 block of East Oak Street. Immediately after Stamets stopped the vehicle Your Affiant positioned his vehicle on the 100 block of South White Street near the Oak Street intersection facing southbound. When exiting my vehicle your Affiant observed a gold Pontiac sedan accelerating northbound on South White Street at an unsafe speed towards Stamets and your Affiant's location.
On 05/30/2015, at approximately 1816 hours, your affiant observed a silver Chevrolet Classic occupied by two males. The vehicle circled the block in a counterclockwise manner, repeating a portion of their route. During this time I located the owner of the vehicle utilizing a search through the Pennsylvania Department of Transportation. The registration indicated the plate belonged to another vehicle and registered to Brittney Fisher, who was not an occupant in the vehicle. I activated my emergency lights initiating a vehicle stop in the 100 block of West Washington Street and while stopping the vehicle I observed the passenger making furtive movements towards the center console.
In summary, on 01/04/17 at 1745 hours, Ofc. Kelley #262 and I were patrolling the area of 5600 W 16th St., at which time I observed a vehicle with tinted windows and no rear registration light (05' grey Acura IL Q336438). A traffic stop was conducted on the vehicle.

On August 27, 2015 at approximately 2154 hrs, I observed a gray 2005 Acura TL bearing NJ Reg# J61FHA was driving 69 mph on 55 mph zone on Route 1 north, made a passing on the right lane and change lane again to the middle lane. When the traffic light turned red on intersection between North Oaks Blvd and Route 1 north, I pull my patrol vehicle 558 next to the vehicle as a the vehicle was slowing for the traffic light and I observed interior light on with a driver who had his hand upon his ear and moving his mouth which made me believed that the driver was talking on his cell phone while driving. This is Mr. Hartshorn’s 1st DUI conviction; there are no others located in his history. Mr. Hartshorn accepts and takes responsibility for his actions that evening. Mr. Hartshorn does not believe he has a problem with alcohol. He states that he had been drinking twice a week but does not see it as a problem since he has stopped before. He advised that he has consumed alcohol since the incident, most recently around two and a half weeks ago. This was discussed with him in great detail. Based on the above information, his history, and what he reported, it is this officer’s opinion that there may be a need for a structured treatment program, and it would be a benefit to Mr. Hartshorn that he obtain an in-depth substance abuse evaluation.
On 04172016 I was behind a vehicle between the 250-300 block of North Washington Ave. Great Bend, KS 67530. The vehicle was a blue Toyota Tacoma pickup bearing KS plate 283FGB. The vehicle was traveling north on Washington Ave. at a slow rate of speed and the brake lights were activating intermitently. The vehicle stopped in the far right side of the north bound lane and had it's vehicle hazard warning lights flashing. The vehicle was stopped for approximately 15-30 seconds. Enroute, dispatch informed me a male party was riding an ATV while it rolled over him, down a barb wire fence, and came to rest on top of him, pinning him underneath it. Dispatch stated he had been like that for an hour now & this incident was occurring at the Tim Pearson Ranch, & a family member of the victim would meet us at the address and escort us to the victim.
