Counselor met with Pt. for his monthly individual session. Counselor and Pt. discussed his progress in treatment, any medication issue, any triggers and acceptance of his TX plan. Pt. signed his Record Service sheet. Pt. indicated that he does not attend the clinic Tuesday, Saturday and Sunday and he likes having more time in his schedule because he likes to take care of his business and different errands. I am writing this e-mail to all agencies in which we have collaborative relationships and/or established MOUs to further encourage communication and support. It has recently been bought to my attention that there has been concern about managing clients when they are in the office for psychiatric appointments. It was noted that staff either dropped clients off at the office, sat in their cars, or were hesitant to intervene when clients became hostile. We are requesting that staff remain with their clients during all appointments and we encourage their participation in sessions if the client is open to their participation. Dr. Agarwal our psychiatrist, is very encouraging of our collaborative approach and would like for staff to attend appointments
The American Cancer Society states that a common side effect of Chemotherapy is “Chemo Brain”. It is defined as the mental cloudiness that is caused by unknown factors in the treatment. Examples of it include trouble concentrating, short attention span, remembering details, slow thinking and processing, and trouble remembering common words. Chemotherapy patients can have Chemo brain from the time treatment begins and can continue for years after which can prevent the patient from doing mental strenuous activities like going to school or work.

Mr. E is an African-American male in his late 40s who is married and a member of the LGBT community. He has several illnesses besides kidney failure that is substantially impacting his health, some of which includes Hepatitis C, HIV, liver failure, and heart disease.
