Introduction In this portfolio I will be reviewing my personal development plan and looking at how I can use these to help me get employed and how I can develop them in the workplace. Care workers and families should be given the information and support they need, and care workers should be given an assessment of their own requirements (National Instiute For Health and Care Excellence, 2014) These are the guidelines I must follow at all times in the workplace. It is necessary to involve the individual in the plan of care and support. Encourage the individual to make choices. This includes their needs, their culture, their means of communication, their likes and dislikes, wishes and feelings, advance directives, beliefs and values, involvement of their family and other professionals. This should be considered and documented. Also, there must be evaluation in assessing effectiveness in the plan of care.
It is important to review care and support plans as people’s needs change. By including the person, their family then everyone knows what is happening and the family can help to monitor mood swings and behaviour. The individual and the family can express their views and preferences and any relevant risk assessments may be done with everyone involved. By monitoring the individual, a decision can be made as to whether the changes are effective and if the best care is being given to encourage independence and promote dignity.
It is important to identify individual’s specific and unique needs so that they can receive the best care possible. Taking a person centred approach is vital, treating a service user as an individual person and acknowledge that

During an initial assessment an individual’s ability and communication methods are established. This is done when an individual arrives into care. Everyone involved in the care of this service user is made aware of their needs and preferences regarding communication and any changes are recognised during reviews and shared with the team to ensure the individual’s needs are met.
It is important in my role that I communicate constantly with my client to understand how she is feeling daily and to let her other carers know her needs and if her needs change at any time. I also need to ask questions and share information with them.
Therefore every care practitioner should endeavour to promote theses rights when dealing with services users and their relatives. Furthermore, it is crucial for service users to understand that any information they give will be with strict confidentiality. It is a legal requirement for health and social care services to keep personal data confidential.

In this report I will be investigating how care services meet the needs of individuals by firstly analysing the needs of an individual using care services. Then I will go on to explain the roles of the care planning process in identifying needs, and explain the features of a positive care practise.
