Hello Mr. Wagner,
On July 22nd I was driving North on Lambert Road in Glen Ellyn about to make a left onto Roosevelt Road. I was in a Enterprise Car Share vehicle that is a 2012 silver Toyota Yaris. As I approached Roosevelt Road there was another vehicle in which case it was a black Toyota. This Toyota sideswiped me as the driver said she was distracted and did not notice my car in the left turning lane as she was in the right lane trying to get over. After looking at her vehicle it appeared most of the damage came from her driver side mirror and right above the rear driver side tire on the black Toyota. Therefore it damaged the passenger side of the Enterprise Car Share vehicle.
The driver, (Rebecca) admitted to being distracted by her phone and accepted responsibility as the police came to make a police report as this is required. As the policeman gathered the story and the rest of the information both the policeman and I looked in the glove compartment for insurance to the vehicle. There was no insurance inside the car. The police officer's name is Mike Jagodzinski of the Glen Ellyn Police Department. Shortly after Mike wrote me a ticket for operating a uninsured vehicle. The nature of the offense was a Rental Car w/ no proof of insurance.

Mike said that he understood and that there was a Enterprise rental facility down the street from the incident. He told me that these matters should be taken care of after receiving proof of insurance from Enterprise. I went to this Enterprise located at 395 Roosevelt Road in Glen Ellyn. After explaining the situation they printed off a copy of Enterprise's insurance. I then took the insurance to 505 North County Farm Road where they said that the ticket could not be handled then because it didn't make it to them
On August 27, 2015 at approximately 2154 hrs, I observed a gray 2005 Acura TL bearing NJ Reg# J61FHA was driving 69 mph on 55 mph zone on Route 1 north, made a passing on the right lane and change lane again to the middle lane. When the traffic light turned red on intersection between North Oaks Blvd and Route 1 north, I pull my patrol vehicle 558 next to the vehicle as a the vehicle was slowing for the traffic light and I observed interior light on with a driver who had his hand upon his ear and moving his mouth which made me believed that the driver was talking on his cell phone while driving. I positioned my patrol vehicle 558 behind the Acura TL, activated my overhead emergency lights and conducted motor vehicle stop.
I relocated, with other officers, to Georgia I-20 (east) and made contact with the vehicle in question. The vehicle was a gold Toyota Camry, Georgia Tag number PZC 8572. I exited my vehicle and made contact with the driver and passenger Bashir Sorey and Ronald Thomas. I explained to Sorey the reason for the stop and asked for his driver's license. On 02/11/2016 at 1436 hours, I was dispatched to Wawa (1060 Delsea Drive) for a report of an intoxicated male fumbling to unlock his blue Ford F-150 that was parked in the parking lot. Upon my arrival, I observed the vehicle in question leaving the parking lot traveling northbound on Delsea Drive. Unit 2 was driving eastbound in the left lane of 167th St. between Orchard Ridge Ave. and Anthony Ave. in Hazel Crest, IL. Unit 2's approximate speed was 35 miles per hour. A large puddle of water had formed from a very recent rain. It covered the right lane and part of the left lane. The driver of Unit 2 noticed the puddle and proceeded to slow down to a speed of approximately 20 miles per hour. The accident occurred as you were driving west on Kearney Street, at Westwood Avenue. You were responding to 4800 W. Kearney to assist two other officers with an active investigation. In that incident, officers were sent to an alarm call and arrived to find forced entry at the business. You were the closest back-up, and volunteered to respond, when the investigating officers made the discovery and asked for assistance.
I arrived on scene and spoke with the reporting caller Loretta Gressens. Gressens related she had her vehicle parked and someone struck the side of her vehicle (Toyota Avalon XLS). The vehicle in questions struck the driver's side of Gressens vehicle. Gressens advised she located a mirror on the street near her vehicle. I noticed the mirror did not belong to her vehicle and was for a Ford Taurus. When asked when this occurred Gressens related she just noticed the damage and it was not like that when she parked the vehicle.
The driver of vehicle 1 stated that he was having neck and chest pain after the collision. I asked driver 1 several times if he want an ambulance, he refused each time I offered. On November 21, 1968, plaintiff Nga Li was driving northbound on Alvarado St. in Los Angeles. She was in the inside lane, and before reaching the Third Street intersection she stopped to make a left turn across three lanes of Alvarado, intending to enter a driveway. While on duty, I, Officer J. Daniszewski, was traveling eastbound on W Westover St. As I was stopped at the intersection of W Westover St and Sawyer St. the suspect vehicle passed me traveling westbound on W Westover St. As the suspect vehicle passed me, I looked in my rear view mirror and could not see a license plate on the suspect vehicle. I turned around and positioned my patrol cruiser behind the suspect vehicle. As I got behind the suspect vehicle, I could not identify a license plate on the back of the vehicle. I reviewed all evidence submitted. Respondent 1’s evidence consisted of a vehicle photo, an ariel scene photograph and Respondent 1 driver’s written statement. The vehicle photo shows left quarter panel damage. It matches the diagram on the police report and does not prove that the Respondent 1 vehicle had almost completed it turn when struck by Respondent 2. The ariel scene photograph shows less detail than the police report diagram and does not support Respondent 1’s contentions. In Respondent 1’s driver’s written statement she says she heard brakes squealing. This contradicts Respondent 1’s contention that Respondent 2 did not take evasive action. I exited my patrol vehicle and made contact with the driver of the vehicle Erica Manning 24 year old female, and the front seat passenger Brent Manning 30 year old male. I identified myself and asked Erica for her drivers license and proof of motor vehicle insurance. Erica removed her license from her billfold and handed it to me. I asked again for her motor vehicle insurance. Erica began looking around inside the cab of the vehicle. Erica began looking in the center console of the vehicle but was unable to locate the insurance information. Erica said she wasn't sure where the insurance is. Erica asked if I would accept the insurance in a digital form on her phone, but had difficulty finding words she wanted to use. Erica began using her phone trying to find her insurance information. Brent began looking into the glove box of the vehicle. While I was speaking with Erica and Brent I noticed they were both talking slowly with mumbled and slurred speech. During our first run Taylor and I noticed that the car was turning way to the left. So in order to fix this we started our car as far to the right of the track as possible. After our first trials we realized that we needed to fix our string. We needed to cut the string we had attached off of our car and make a new one. When we made a new one me made it a little bit longer than the car and only hot glued it to the hook, not to the frame. Our new string had a loop in the end of it so we could put that on the toothpick. So now we understood that the toothpick did have a real purpose and we couldn’t glue the string to the mousetrap. After we made these changes we ran our car three more times.

The black female driver ignored my command and entered the residence. The black female driver exited the residence a few seconds later with two black males. I approached the black female driver and one of the black males as they walked towards me. The other black male stood by the front door. I informed the black female driver why I conducted a traffic stop. I told her I needed to see her driver’s license and proof of insurance for the vehicle.
