I found this particular portfolio more challenging than previous ones. The main reason for this was the fact that I was the required to actively take part in the assessment, planning, implementation and evaluation of a patients care within the service. Doing this came with responsibility that I had not had in previous placements.
I THEN WALK TO THE KITCHEN DOOR AND SEE IF THE COOK HAS EVERYTHING READY AND PREPARED. THE COOK IS WORKING OFF A PREVIOUSLY AGREED MENU (AGREED BY THE RESIDENTS) ONCE EVERYTHING IS READY, I PUT ON A CLEAN APRON (GREEN-FOOD/CATERING) AND GLOVES (BLUE). I THEN PROCEED TO TAKE THE TROLLEY OF FOOD TO THE DINING ROOM. EACH RESIDENT IS REMINDED OF WHAT IS BEING SERVED FOR THAT MORNING AND IS ASKED WHAT THEY WOULD LIKE AND IS GIVEN WHAT THEY HAVE CHOSEN. ONCE THE RESIDENTS HAVE EATEN THEIR CEREALS AND SECOND WARM BREAKFAST AND DRANK EITHER THEIR CUPS OR TEA,COFFEE OR JUICE EVERYTHING IS COLLECTED AND PUT NEATLY ON THE TROLLEY AND TAKEN BACK TO THE KITCHEN.I WILL THEN REMOVE MY GLOVES AND APRON AND AGAIN WASH MY HANDS. I THEN HELP THE STAFF ASSIST THE RESIDENTS BACK INTO THE LOUNGES. I WILL THEN ASK ONE MEMBER OF STAFF TO WIPE THE TABLE MATS OVER AND TO HOOVER THE CARPET IN THE DINING ROOM, AND TO RESET THEM READY FOR LUNCHTIME, USING CLEAN UTENSILS. I ASK ANOTHER MEMBER OF STAFF TO CHECK ON THE RESIDENTS WHO ARE EITHER STILL ASLEEP OR ARE HAVING BREAKFAST IN BED AND SEE IF THEY ARE READY TO GET UP NOW AND TO ASSIST THEM AND I ASK THE THIRD STAFF MEMBER TO WATCH THE RESIDENTS IN THE LOUNGE. IF THERE IS A MESSAGE LEFT FOR ME TO RING G.P'S OR ARRANGE TRANSPORT I WILL DO THIS NOW BY GOING INTO THE OFFICE AND GETTING THE CARE PLAN OF THE RESIDENT I NEED TO CALL THE G.P OUT FOR (OR BOOK TRANSPORT), I RING THE SURGERY AND EXPLAIN WHO I AM AND WHERE I AM CALLING FROM AND THEN
For me, the most exciting time of the year comes in late August. It is the Middlesex County 4-H Fair, in which I give not so much my weekend, but my life. As the title implies, the fair is put on by the 4-H organization. This youth organization uses agricultural education and community service to teach important life lessons. I joined 4-H when I was eleven years old and it continues to change my life. I have learned communication, leadership, and even my love for education through the wonderful programs and dedicated volunteers.
A common goal all healthcare providers share, is the desire to provide excellent patient care. The delivery of care is constantly changing in healthcare, however, the patient will continue to remain the focus of care. The success of nursing care thrives off the ability to fulfill patient needs and to maintain patient safety and satisfaction. When patients are admitted to the hospital, their need for an increase in their level of care and attention, due to the decline in their health status, and inability to preform normal daily activities of daily living. The loss of independence places the patient in a vulnerable state of mind, causing the individual to rely on members of the healthcare team to assist with basic self-care needs while in a stable and well-organized environment. A structured environment can be accomplished through the practice of hourly rounding on all patients.
The second thing that i gained from the fair was that I have a chance find out a new place accepting volunteers. It is the Public Health Department at Madera . It is a good opportunities for me to get involved in the community and gain more experiences for my future career. Besides that, i also learned about "Fresno Diabetes Collaborative", they have meeting every month in Clovis area. They are current collaborative with so many different system such as Adventist Health, Anthern Blue Cross, Valley Children's Hospital, etc. Each month they are going to gathering together and working on the same project about Diabetes, helping each other in career and sharing

All staff meets briefly to make everyone aware of how many patients are booked; how many appointments are confirmed; and how many appointments are available. Staff is also informed of who will be out, who will be late, and what things need to be done if time permits, so that priorities for the day may be established.” (Schuman, August 2015)
I was with my mom, and I wasn’t in school, yet I just got done with working at the fair. I spent a night at my mother’s house so that I can spend some time with her. When I woke up the next morning, I thought to myself y don’t I take my mom out to eat. So I got up from my bed and went to her room. Knocking on her door, I ask her if she wanted to spend a day with me. She got up and opened the door telling me that she would love to. I want into my room and found some clothes to wear, finding something cute I want to her room and got into her shower. Getting out of the shower getting dressed I asked her if she was about to get in the shower, she yelled out “YES.”
I went to Free Clinic on Wednesday in part to get away from all that is going on currently, and I quickly realized that I couldn’t escape the realities many face today in America.
One of the major steps to take is to explore the patterns of the staffs and how the interact and relate when it comes to patient care, this will help in establishing a plan that will help the hospital provide the best care, decrease in falls, pressure ulcers and pneumonia
The phone was ringing as I stepped into my house. My mother picked up the phone without realizing she would shortly be sent to her knees. She yelled for me to get into the car and we rushed into the hospital. As hectic as the situation felt, time seemed to pass by very slow in that car. We finally arrived to the hospital to find my father nervously talking to the doctor. I learned that my brother had been brutally beaten up by a couple of teenagers on his way home from school and, as a result, had lost most of his teeth.
I was born on September 20th, 1997 on the coast of Virginia Beach. Now living in South Carolina I am 19 years old and living a very healthy and eloquent lifestyle. As I filled out the Real Age questionnaire I encountered many questions that made me think if my diet and health were flawed, while other times it seemed as if I was the pinnacle of health. I found at the end it had me down as a 16-year-old teenager. This three-year difference in what my age and what Real Age had put me down for really opens my eyes about how well of a healthy lifestyle I am living, and motivates me to continue living it to the best of my ability.
Dietary employees that bring the patients food tray should also make sure that everything is within reach. While they are in the room they should also ask if the patient needs to go to the bathroom, if they are in pain, and do they need to change their position before leaving the room. At that point if the patient is needing anything that they cannot perform for them they should inform primary nurse. Housekeeping while in the room cleaning should do the same as dietary, with both housekeeping and dietary make sure that the patient are ask about the 4P’s there should be a decrease in the call lights.
My trip was to go to CMMC Wellness Center; unfortunately the plans didn't go as well. So we had to stay at LHS and had a guy came and talk to us about what he does and our job shadow. Ryan Westleigh attended Bates College and eventually became coach. He has played basketball, football, soccer and ran track. Westleigh talked about his career and what he does as an athlete coach and their latest scores. He didn’t really talk about the career but I believe the three significant academics skills would be the ability to communicate by way of written expression, the ability to recognize a problem and think critically to solve it and the ability to use technology effectively. All these academic skills can be used in coaching at all times.
The purpose of our health fair event is to educate and encourage older adults to remain active in their daily lives. Physical health is beneficial throughout one’s entire lifespan. Incorporating regular exercise into your daily routine will prevent chronic disease, improves their overall mood, and lowers their chances of injury (Aldwin & Gilmer, 2013, Chapter 5). As we go through the aging process, our bodies require us to maintain a healthy lifestyle. Moderate exercise is favorable for people of all ages. Elderly people living with chronic illnesses can also participate in moderate physical activity. Medical conditions, such as dementia, heart disease, diabetes, and high blood pressure can all be improved by including regular exercise into an individual’s routine (Chronic Illnesses, PowerPoint).
