On 12/29/17 I met Ms. Becquette at the office of Dr. Saleh. She arrived ambulating with crutches. She had on an ankle brace and a full leg brace. The pin sites have all healed. X-rays done today showed that there is bone healing, the fractures have not fully healed yet, there is also some osteopenia, which we would expect from the dis- use of the bones. She can only bend her knee to 26 degrees, the ankle she cannot bend it is dropped and contracted, and she is able to wiggle her toes. The numbness she has had to the leg and foot is gone. The EMG has not been done yet because of Mom's schedule. She said she would call and make an appointment soon. Physical therapy is making some improvement. Dr. Saleh said he wants her to work really hard in therapy, he said it would be a last resort to go in and release the adhesions to the knee and the Achilles tendon for the ankle. Dr. Saleh said he feels she can work through this. She is allowed to bear weight as tolerated. The new brace has not arrived yet and is supposed to be in next week. We did address the attendant care and Mom was hesitant to decrease to 8 hours a day. Ms. Becquette will be going back to public school on 1/19/18. May 18th 2014 Mr. Beaird fell and broke his hip. This followed hip surgery performed by Dr. Baker and 21 day rehabilitation form Cordova NH. Before his 21 days was up Mr. Beaird tried to break out of NH by running his scooter though the front door at the NH. When the scooter hit the door, the door opened and Mr. Beaird rolled onto the front porch during this altercation, his foot was injured and the ambulance was called. On 04/14/2016 at 2315 hours your Affiant was dispatched to 109 E. Center St. Apartment Upper for an overdose. Upon arriving on scene EMS was tending to the overdose patient. Your Affiant spoke with a female in the apartment who identified herself as Tiara Pleva while Stamets spoke with the male who lived in the apartment Aaron Campbell. Pleva stated Conrad shot up with heroin. Your Affiant asked where the heroin if any left and empty baggies are, and needle Conrad used. Pleva stated she does not know where any of the stuff is.
DOI: 07/10/2012. Patient is a 67-year-old female licensed vocational nurse who sustained work-related injury to multiple body parts when she tripped over a cord and fell onto the floor while leaving a patient’s room. Per OMNI entry, patient is status post right total knee replacement on 1/10/2015.
Many patients admitted to the stroke and orthopedic rehabilitation unit have impaired physical mobility. The length of time in rehabilitation is ten to fourteen days. Many times nurses, patients and family members form bonds that last long after the discharge. I recently had the opportunity to take care of a patient I will never forget. Mrs. C was admitted to the rehabilitation unit following recent hip surgery. She is eighty years old and had fallen raking leaves in her front yard. Mrs. C has a history of hypertension, arthritis and gout. Medications include aspirin, metoprolol and allopurinol as needed. Prior to admission Mrs. C lived independently and has two children who checked on her routinely. No cognitive or mental deficits are noted. I'VE BEEN SUPER EXCITED FOR THIS TO COME OUT!!! For those who don't know this is one of my anticipated of the year (you can check out my list if you haven't already, I posted it in September). But since this is the last book of the series I don't feel the need to write a very long non-spoiler review. I extended some of the restrictions including no work as I want patient primary laying horizontal. She can sit for three hours a day in her classroom but other than that, I am going to have her use a Rollator for walking in the unit short distances, wheelchair long distances. With the prednisone she may get better significantly faster so this may change over the next two weeks. I also fitted her with a lumbar support which allowed for standing and walking to be far greater tolerated. She was able to stand and walk with the Rollator with the brace and had a difficult time without it. She can use this brace as need be. On June 10th 2016 I woke up and went out to start my day. Since it was summer I usually spend my days swimming or at home playing in the yard. That day I decided it was a perfect day with excellent weather to spend my day at the Plymouth pool. It was a great day to lay out in the sun and get a sun kissed glow, but most of all to hangout with my friends. I would usually spend my whole day from noon to 6:00pm at the pool, but this time by surprise my mom picked me up early and told me my brother and his girlfriend Renee wanted to take me out to Chicago with them. I was told I was going to go to a carnival or out to eat in Chicago, but my mom knew all along where I was going. She told me to find a shirt with dark blue on it but I never knew

The sutures to the outer and inner aspect of the ankle were removed. She feels pressure and rubbing from everything on the incisions. Dr. Nwuso told her to stop using the crutches, do not use a cane, and let physical therapy wean her out of the cam boot. When asked for work status, Ms. Iversen said she could not work with a boot or a limp. Finally Dr. Nwuso wrote for her to start with restrictions and work no more than 8 to 14 hours per week. He will advance them as she deems necessary. She will follow up in 6 weeks, on 5/1/17. I asked how her back was doing and she said it was fine. She has completed physical therapy and is doing a home exercise program. She is worried that her cervical spine was damaged. She has a prior issue with her neck. I asked her if her neck is worse or the same than it was. She said it was the same but worries something
