On 010/05/2016, I, Chad Agnew, was working as a patrol officer for the Wichita State University Police Department, in Wichita, Sedgwick County, Kansas. At approximately1415 hours, officers were dispatched to the Rhatigan Student Center for a suspicious character. Dispatch notified all officers that they were still gathering more information at the time but the suspicious character was a black male with crutches. They also said the subject was yelling someone had a gun.

Upon my arrival as I approached Cell 15 I noticed large amounts of blood on the floor inside the cell as well as the words "I'm sorry" smeared in blood on the back wall of the cell. Inmate Whitehead, Richard MCSO Booking Number T337608 was lying on his bed unresponsive. Moments later I entered cell 15 with Officer Shoemaker B3008 to help remove Inmate Whitehead from his cell and place him on a gurney by lifting him using the sheet from his mattress underneath him. On April 05, 2016 at approximately 1518 hours, I was dispatch to 2004 Pine Street in reference to a death investigation. Upon my arrival, I met with Detective Campbell who was the lead Investigator at the scene. Detective Campbell stated that the victim neighbors reported a strong smell coming from the victim’s (Cortiz Lazarz) residence. The neighbors called 911 and Officer Volk was the first responded officer on scene. Defective Campbell and officer Volk stated that the victim located on the living room floor, fazing the south from the residence. Detective Campbell requested I process the scene for evidence and documentation.
