There were also two adult men in the backyard of the neighboring house. Upon approaching the front of the address, I made contact with a woman claiming to have reported the incident. She stated her name as Mary Sue Jones. I observed that Mrs. Jones had wounds on her face as well as cuts along both of her arms. Mrs. Jones was visibly shaken, she explained that her husband, Paul Jones, had thrown her to the ground and punched her in the face. Mrs. Jones also warned that Mr. Jones was drunk. After the initial contact I was shown into the home where the alleged incident occurred. Upon entering the home, I was lead to the living room, where I discovered a large coffee table which was shattered and a table lamp lying on the floor beside the broken glass. I observed three empty beer bottles as well as a half bottle of rum lying on the couch. Mrs. Jones indicates that she obtained the cuts from the broken glass on the coffee table and that Paul had hit her in the face. I arrived on scene at approximately 02:11 hours. I approached the front door and the victim, Hollie J. McIntosh (F/W, DOB: 07/26/1988) opened the door and let me inside. I inquired with McIntosh what happened. McIntosh stated she was shoved and choked by her boyfriend, James Dean Carvell (M/W, DOB: 01/25/1988). I observed a red mark on McIntosh’s neck. On August 23, 2014 Sgt. Lavelle was assigned to the 83 Precinct. She was present for duty at 0800 hours as the domestic violence Sergeant and she conducted several home visits. At 1450 hours she was post changed to the 83rd Precinct desk and she assumed the duties of the 83 desk office. Sgt. Lavelle remained as the Desk Sergeant for the entire tour. I presented Sergeant Lavelle with a copy of the command log entry for August 23, 2014 depicting the validation of two arrests at about 2040 hours when Mr. Carlton Smith
On Sunday November 24, 2016 at approximately 16:02 hrs. Officer L Wilkerson and I Ofc. R. Holcomb were dispatched by dispatcher B Coleman to 304 Hopkins Drive in reference to a disturbance. Upon my arrival I made contact with Ms. Shawntavia Lucky (complainant) who stated that her son Mr. Jousha Lucky (victim) got into a physical altercation with two black males.
I then went through the family violence packet with Odelia Butler. Odelia Butler told me the during the physical altercation Rodeny Butler struck her on the right shoulder blade region causing her physical pain and injury. Odelia Butler then filled out a voluntary statement form which will be attached to this report. Odelia also stated she did not want an emergency protective order and was not going to the hospital. Photographs where taken and will be attached to this report, while taking photographs I did not locate any discoloration to Odelia Butlers right shoulder blade
I then proceeded to enter the residence and noticed a female was visibly upset and was crying. I identified the female as Mrs. Gloria Lipscomb Plato (Victim) and her daughters Marissa Dawn Plato (Victim) and Kara Plato. Mrs. Plato’s face was red and she had a cut on the inside of her bottom lip. Mrs. Plato’s neck and arms were also red. Mrs. Plato stated that
Washington showed Sabastian’s bedroom. I could see that the window screen sitting upright on the ground outside of the window. The window screen did not appear to be damaged at that time. Washington said that nothing appeared to be disturbed. Washington said she wanted the juvenile males prosecuted for attempted burglary and possibly damage to her window screen. I then gave Washington a crime victim’s information card with the case number and my name.
While on scene I observed blood droplets in the kitchen, dining room, Living room, laundry room and a long hallway leading into the residence from the backyard. Once outside of the residence I observed further evidence of a physical confrontation, including additional blood spatter and droplets scattered throughout the yard near a burning fire where it appeared, due to multiple empty alcoholic beverage containers the subjects had been drinking.
Anna was upset and yelling at Gilbert inside the residence. Shiela left the residence and Anna followed her onto the porch. Anna pushed Shiela almost off the porch so Judy pulled Anna away from Sheila to keep the situation from escalating. Anna left the residence and came back twice. Judy did not see any injuries to Anna until she came back the second time. Judy does not know how Anna received her injury. I checked Judy's hands for any signs of trauma to her hands or fists from striking something and did not observe any marks. Judy denies causing injury to
Mason walked me to her vehicle and I observed that all fours tires on the vehicle had be slashed. All four tires were now flat and they had clear stab marks in the outer walls. I also observed that the windshield to the vehicle had been smashed inward by a round object. I located
Mr. Foster stated that the victim, Mr. Addison Rae Thomas and several of his friends came to his house because they got into an argument about Mr. Thomas calling Mr. Foster’s wife on the phone. Mr. Foster Stated that they had a few words with each other before Mr. Thomas began to advance further in Mr. Foster direction in an aggressive manner.
Plaintiff Brian Martin attended a high school graduation party hosted by Defendant Lee Martin. The majority of guests were between the ages of seventeen and twenty. There were two kegs of beer available at the party, as well as other alcohol provided by the guests. One guest, Matthew Marciano, had a history with Mr. Martin. During the party, a fight ensued between Marciano and the plaintiff’s friends. Marciano left the property, but later returned with Chijoke Okere, who was carrying a baseball bat. Okere struck Mr. Martin on the head. The defendant went inside her house and locked the door, refusing entry to those who were trying to help the plaintiff. Mr. Martin suffered considerable brain damage from the injury.
The reporting party (RP) stated on 6/8/16 client Jacqueline Cazarez and her mother (a name not provided) met at the office. The client's mother discussed the reason she no longer wanted her daughter to attend the day program. The mother disclosed that strange things were happening to her daughter. According to the mother, last week the client arrived home with a scrape on her knee. On 6/7/16 the client arrived home with big bruises under her right arm and right leg. The bruises were observed by the RP. The mother is not happy with the treatment her daughter is receiving at the day program. Subsequently the mother is aware that her daughter has behavior issues and agrees that she is not perfect. The mother disclosed that the transportation driver

A 32 year old woman was admitted to the Trauma Intensive Care Unit following a motor vehicle accident; she had multiple injuries and fractures, with several complications which continued to develop over the first couple of weeks. The patient rapidly developed Adult Respiratory Distress Syndrome, was on a ventilator, and was continuously sedated. Shortly after the patient's admission, her parents were contacted and remained vigilant at her bedside. The parents reported that the patient was one month away from having her divorce finalized. The patient's husband was reportedly physically and emotionally abusive to her
