Abstract
Providing patients diagnosed with Congestive Heart Failure effective teaching can eliminate reoccurring hospitalizations. Patients are discharged with CHF and readmitted within 30 days. The information provided will examine the process of enhancing patient knowledge and provide additional resources essential for effective health care management. Research evidence provides data that proves patients who are diagnosed with CHF needs a variety of health care needs during admission and after discharge. The proposal will display an evaluation plan, implementation plan and a dissemination of the evidence.

Hospitals nationwide have been striving to reduce the rate of patient readmissions. Both the federal government and private insurers are tired of picking up the tab. In a 2009 study in the New England Journal of Medicine, researchers estimated that a year's worth of unplanned re-hospitalizations cost Medicare alone $17.4 billion. Congestive heart failure is a particularly big target, as one in four patients end up back in the hospital within 30 days of discharge. Starting in the fall of 2012, the government will cut Medicare reimbursements for hospitals with higher-than-expected 30-day readmission rates for heart failure and two other conditions: heart attack and pneumonia (Avril, 2011).
Solution to the Problem
Recently the hospital had a meeting about the way the facility would be reimbursed due to hospitalizations that occurred within a thirty day readmission connected to the same diagnosis. The educators were forced to evaluate how staff could improve in eliminating infrequently readmissions. The lack of information given and understood by the patient guarantees a readmission due to the nurse not fully understanding that for patients with CHF to avoid readmission due to noncompliance nurses and health care staff must ensure that the…
