According to the RP the resident was treated in Eisenhower Medical Center located at 39000 Bob Hope Dr., Rancho Mirage 92270 (760) 346-7772 in February 2017 and release on 2/26/17 to Brookdale Mirage Inn's Skilled Nursing facility. The RP stated the gangrene was discovered during physical therapy by the physical therapist.
