Successful CC =1 CM called Karen Williams (DCF Coordinator) at 856-339-6726 to inform her that youth is missing from Daytop program. CM explained to DCF Coordinator that case manager left a message on CM voice message on 1/26/17 at 7:10pm in regards to youth being missing. CM was informed to use code EL110; violation of probation.
On 07/06/17 at approximately 0817 hours, Officer Flick I (Ofc Paipoovong#24) responded to 3656 Hoffman Rd#304 on a welfare check complaint. The complainant/mother, Evelyn LOUISE Vermeersch dob: 02-04-1940, requested that an officer check on her adult daughter, Carol Lynn Jacobson dob: 10-12-1966, who is also wheelchair bond because she had not heard from her since yesterday.
CM emailed Andrea Orlando (Daytop Program Coordinator) a copy of Dre’quan’s (youth) updated biopsychosocial (BPS) and court order. Ms. Orlando reported the clinical director will review youth’s documents and will determine admission date. Ms. Orlando will continue to keep CM updated regarding youth’s status. CM provided CM’s contact information.
CM spoke to Sergio Paredes, CMO Court Liaison regarding an update on Dre’quan (youth) OOH treatment status. CM informed Mr. Paredes that the Clinical Director at Daytop is still reviewing the referral documents and Daytop have not contacted CM regarding an update on youth’s status yet. Mr. Paredes provided CM with Honoria Forte (Youth’s Attorney) for CM to follow-up with attorney regarding this matter.
Client reported to CM she has a new granddaughter and tomorrow she will be spending the day with her. Client report no community support. CM enrolled the client to Adult Recreation Center.
WSDOTMF scholarships were awarded to two individuals in 2015; Nellie Williams and Tristin Richards. Nellie Williams is pursuing her education at Centralia Community College. Nellie will be pursuing a transfer degree with plans for an eventual four year degree at an in-state university. Tristin Richards is pursuing his education at Peninsula College. Tristin will be pursuing a transfer degree with plans for a business and marketing degree from an in-state university.
On Friday 02/23/18 at 1447 hours I was dispatched to a juvenile problem at 3340
On July 6, 2015, I was assigned this case to follow up. This case involves Mr. Tavon Lanier a fourteen year old male. Mr. Lanier’s mother, Ashauta Lanier reported her son ran away on July 4, 2015. On July 7, 2015, I spoke to Mr. Don Earlington Intake Officer of the Court Service Unit. He confirmed an active detention order for Mr. Lanier. Mr. Earlington confirmed previous runaway events and additional incidents in Maryland where Mr. Lanier had taken a handgun to school. Mr. Lanier’s current Probation Officer is Macalister.
Reporter stated the following: He is in DHS custody. He (Issac) came to me on October 20th. He (Issac) was in foster care. He (Issac) told me this the other day and it was about 2 or 3 days ago, “I guess”. I have never reported it. I called today because Ms. Judith with MYPAC came in my home today and told me to make a report. Per intake worker (Gail) was on the phone also. Per Ms. Judith I was his (Issac) therapist with Youth Villages.
MSTT was unable to work with Jose and Juminia on this weeks goals because youth runaway from home on Friday evening. Youth remained out in the community for five days before he was find by his probation officer. Youth will be having a hearing to determine what going to happen next with him. MSTT will collabrate with Juminia and Ricky (JPO) to explore the situation and review all the outcomes. MSTT will continue to reach out to Juminia and Ricky during this time until a final decisoin is made by the court.
On March 28, 2016, I was assigned this case to follow up. This investigation involves Ms. Ashlyn Cruz a sixteen year old T.C Williams High School student who is a frequent runaway. In the last couple of months I have been assigned multiple runaway cases Involving Ms. Cruz. I have been in touch with Mr. Locke of the Court Services Unit and her probation Officer Pamela Nieva.
Karen Horney was born in Blankenese, Germany, on September 16, 1885. She attended medical school and began studying psychoanalysis. Consent: The child and family team is in agreement that Out of Home Treatment is an appropriate treatment option for the youth at this time due to youth behaviors that lead to numerous arrests and violation of probation. Youth needs to address his mental health, substance use and behavioral issues in the community and in the home by engaging in safe behaviors. The youth has demonstrated a lack of impulse control, oppositional behavior towards rules and regulations of the home, school, and community prior to DAP Bed. The program noted Dre’quan has been staying out of trouble and requested for youth to continue with treatment at Florence Klemmer House/DAP Bed. Belonging to a certain group is a natural experience in the lives of individuals. On December 15th, 2016, Officer Wisdom was dispatched to Kids’ Space Child Advocacy Center in Muskogee, Oklahoma to take the initial police report. Once there, Officer Wisdom made contact with the Department of Human Services Case Worker, Amber McKinney.
