A. Developing a caring relationship with a client is an important aspect of case management. When meeting with a customer, the case manager needs to be cognizant of his or her verbal and body language clues are congruent. The initial engagement will set the tone for the future relationship with this client. Allowing the person to vent will provide clues about how he or she relates within the environment, including with family and friends. B. Ballew and Mink (1996) stated the importance of documenting and formulating a client “service plan” is critical p.21. This record would aid in maintaining order and planning of short and long-term activities. Sharing this with members of the management team facilitates collaboration among each discipline encouraging the attainment of set goals.

The family or significant others brought into the case as soon as possible aides in the plan outcomes. They can reinforce the program and provide needed guidance and support to the client, to help improve the customer's self-confidence. Besides, they provide a second set of eyes for critical review of their loved one's progress within the plan.

Case management is a continuous balancing act of judgment calls, making ethical choices, getting along with coworkers, and following legal protocol. It is the duty of the case manager to know how to develop a relationship with both clients and coworkers while still maintain their professionalism. The healthcare professional must always leave their personal opinions at home and provide the same quality of care to all their clients regardless if they have different views, religion, sexual orientations, or cultures. The case manager needs to ensure that the client’s concerns will be put first no matter what, and should also clarify that the as the professional he or she will do all that is necessary to provide the best quality of care to their client.
In social work practice, client assessments are important because they allow social workers to gather information from clients. This information helps the acting social worker better understand clients presenting problems. The assessment also helps social workers evaluate a client’s progress and evaluate the effectiveness of interventions used. In this paper, a multidimensional assessment will be performed on a client I have been working with at my internship placement, Urban Ministries of Durham. Once all relevant information is presented, and intervention and treatment plan will be discussed.
It is necessary to involve the individual in the plan of care and support. Encourage the individual to make choices. This includes their needs, their culture, their means of communication, their likes and dislikes, wishes and feelings, advance directives, beliefs and values, involvement of their family and other professionals. This should be considered and documented. Also, there must be evaluation in assessing effectiveness in the plan of care.

Lastly, key bodies of knowledge will be identified and applied to the case study.
Once the client’s needs have been established and the resources have been located the case manager must constantly review the progress of the client and determine if continuing case management service is needed. Terminating the relationship is never an easy process; the case manager must carefully create a termination process depending on the client’s level of dependence in him or her. If a client is heavily dependent on the support of the case manager, the need for a gradual decrease in the amount of contact with the client may need to occur before termination.
These are the three models of Case Management that are used The role-based case management, the organization-based case management, the responsibility-based case management. The main reason for the difference of the three is for servicing the clients the best and best taking care of their needs. The role-based case management main goal when working with there clients is make sure their clients is receiving the services in which their client needs.
