Reason for Referral: Jim was referred for a learning disabilities evaluation, vocational evaluation, and a general personality assessment to aid him in making plans for his future following his honorable discharge from the military. Background Information Jim is a 25-year old, single, Caucasian male and currently resides with his mother. Jim was recently honorably discharged from the military where he worked in a variety of skilled labor positions. Prior to joining the military Jim spent two years doing manual labor. Jim was in special education classes during his early educational years; however, he finished high school in regular classes. During his time in the military Jim received speech therapy and was hospitalized twice for depression.
After college, Jack decided to enlist into the military. He wanted to serve his country. Mr. Brown has severed in the military for 25 years, combat training for the last 5 years and has 3 deployments. He was planning on retirement in about five more years, before this last tour and injury. Mr. Brown was married for about 15 years. He was sole provider for his household. They lived on base to cover expenses and moving to different station as he was assigned. His wife was having a hard time with his last deployment.

This position would enable him to provide for his family and help America’s oppressed and disenfranchised population. Despite his sacrifices for his country and meeting educational requirements, James found himself blacklisted from the profession for not having adequate experience in the field. Currently James is attempting to create a new opportunity in his life by obtaining his Master’s degree in Social Work. He hopes to become a social worker for the United States Army. William Thomas is a 34-year-old African American man that lives in Killeen, TX with his wife, Sandra and his son, Timothy (9 years old). William entered the Grace Christian Center on June 23, 2016. William suffers from PTSD as a result of his combat deployment to Afghanistan in 2014. William served in the United States Army for six years, He enlisted in 2010 and exited the military on an honorable discharge February 10, 2016. During William service, he achieved the rank of Sergeant and served in two combat deployments. William has no known drug allergies but said that he refuses to medication which is why he is here to see Grace Counselors instead of the VA. William loves to shoot guns, exercise and play the video game call of duty. William has been having anger outbreaks and cry spells since he left the service just four months ago. His wife, who escorted him to the church said that they are constantly fighting over stupid things and that he treats her like he’s her soldier. William keeps trying to explain himself but he says his wife doesn’t listen. He’s not treating her like a soldier he 's just trying to point out some things and he doesn’t feel respected therefore he feels the needs to raise his voice and through a tantrum.
John is a married man, who serves in the military service. It was Jon’s wife who encouraged John to seek counseling. John is having difficulty dealing with his life, he feels it’s worthless. He has no motivation to stop jeopardizing his job or marriage, even though he is aware that his actions are causing complications in both his work performance and in his marriage. John toured in Iraq, and decided two months ago that he no longer wanted to be affiliated with the military service, because of stories he heard about the deaths related to wars in Iraq. When he returned home from his tour six months ago, he was happy to be home, but slowly started losing motivation to go to work. He started showing up late, and not fulling his duties at work. His wife is at this point tired of John’s behavior and advised him to seek therapy.
Jim had a passion for birdwatching, and during his time he was given a career to watch birds, and pursue his dream, which allowed him to express what he loved and this is what he filled his life with. This was the case until World War 1 came about, and the propaganda started to arise. Due to the false advertising of war, many people decided to enlist, and Jim felt pressured to follow the crowd so he also enlisted in order to not be seen as a coward. Jim’s decision to enlist went completely against what his aspirations were in life, and in doing this he unnecessarily shortened his own life by not making the right decision. Being a year 12, I am in the brink of my schooling and there are many decisions to make in regards to my future.
In the future, I will use referrals in my classroom when a student is being blatantly deviant and I have exhausted everything in my power to try and contain the situation myself. I will also use referrals when a student hurts another student. To me, referrals are the last form of discipline. These are not something to be taken lightly and since they require the assistance from either the Principal or Assistant Principal, I will only use them if/when needed.
He has a lot of qualities that make him a lion, golden retriever, otter, and a beaver. He has a really complex, but amazing personality. Jim can be very intimidating and all up in your face, but he is also very caring. Jim is hard on the outside, but soft on the inside and can be hurt very easily. Jim is also someone who loves to start projects and works very hard at them. When he is working on something he is very thoughtful about what he is doing and likes to get it just right. He is so creative and is very talented in all sort of areas. God has blessed him with so many talents. He surprises me a lot and I’m always amazed at what he can do. When he is motivated he can do anything and he excels at it, but when he is unmotivated it can all fall flat. Finding what keeps him motivated can be a struggle because he wants to make sure he can benefit from it. He likes to debate topics he is passionate about. Jim never likes to admit when he is wrong, and will fight till he is blue in the face. His personality type seems to be all over the place having aspects of each one.
