O: Left Pointer Finger Nail: A Blue colored discoloration, tender to palpation; Left pointer Finger distal phalanx 1 cm in length purple discoloration, small, pin tip size open area, bleeding has stopped. mild edema

O:Left Wrist: no edema, no discoloration, full ROM, no impairment of the NVS, radial and ulnar pulses +3, pea-sized bump palpated in the dorsal radial border that is firm and stationed and causes pain with pushing on it or extension of the wrist.
Cardiovascular Assessment: No visible pulsations, no heaves or lifts. Apical pulse present in the fifth intercostal space at the left midclavicular line. Auscultation of apical rate 62 beats per minute, normal rhythm regular S1 - S2 heart sounds present. Pulsations present when supine and disappear at a 45 degree angle position. Extremities are brown color without redness, cyanosis, lesions or varicosities bilaterally. Temperature warm bilaterally, Allen test was negative. Homan’s sign negative. Carotids: +2 and present bilaterally. Right Radial +2, left radial +1 , Right Brachial: +2
Pedal pulse are usually decreased or absent during this stage. The second stage of PAD is claudication also known as the “limb” stage. The fourth and last stage of PAD is the necrosis or gangrene stage which comprises of ulcers and blacken tissue on the toes, forefoot and heel with a gangrenous odor. S: TM is here for follow up of her Right Knee Contusion. Tm reports her current pain is 6/10, sore ness that is localized to her right patella area. TM report she did ice her right knee at home last night and she took her ibuprofen this am. She may go see per PCP. TM was informed, if she goes and sees her PCP, it becomes personal condition and TM would not be treated by HMMA Medical Clinic. TM reported that wasn’t a problem with her since she only had one body. Her personal feeling is she needs to go home today and return to work tomorrow since she is still sore from the contusion that had occurred yesterday. A bruise is a kind of injury that is caused when part of the body hit something. The impact causes capillaries under our skin to be damaged. This causes blood to get into the surrounding tissue. Bruises often hurt later. They are usually not dangerous. A bruise can be seen a day or two after the impact. It is usually blue or purple in color. Then, it turns yellow and slowly goes away. The changing colors comes from hemoglobin. Hemoglobin contains red blood cells. It also contains bilirubin and biliverdin. Bilirubin is yellow. Biliverdin is green. These two things combine, then breakdown to make the diferent colors of the bruise. It takes between one and two weeks for a bruise to heal completely.
Foot injuries are very common in athletics as well as in everyday life. It’s very debilitating to have a foot injury since we use our feet in all of our daily activities. Research published in "Medicine and Science in Sports and Exercise” indicates that the average adult takes between 5,000 to 7,000 steps a day. Some sports require the most dedicated athletes spend multiple hours a day pounding their feet on the turf or pavement. Most injuries that occur in the foot require a person to try and stay off of it or completely immobilize it. Since this is very difficult for a person to do, a large percentage of foot injuries often have a very high chance of reoccurring. The severity of some injuries that can be deceiving as well. Often times a nagging pain is ignored and eventually becomes a much bigger problem.
