On March 29, 2017 at approximately 8:11 PM I, Deputy Bowring, was dispatched to 2896 FM 2088 Quitman, Texas in reference to a subject found hanging in a tree but was still breathing.

Central Wisconsin Center (CWC) is an organization established in 1953 by the State of Wisconsin to provide residential and short-term treatment to people with intellectual disabilities. It is one of the three facilities used by the Wisconsin Department of Health Services to take care of disabled people in the state. The organization was recognized by the state’s legislature in 1953 and then opened to the public in 1959. Today CWC have over eight hundred employees with a yearly budget over fifty million dollars and occupied 100-acre campus located at 317 Knutson Drive, Madison, Wisconsin.

On 5/13/2015 I met with Dr. Navato at Centerpoint Medical Center. Dr. Navato was familiar with the complaint as she had submitted a narrative response to the BHA in February of 2015. The allegation was that Dr. Navato did not honor a (Do Not Resuscitate) order.

Our nurse practitioner will be attending a three-day long fall risk seminar in Los Angeles in November. The cost of the seminar is $2,300.00 not including the flight or room and board while she is there. We have budgeted out $1,000.00 for the round-trip flight, room and board, and meals while she is there. We believe this will be an acceptable amount for all the needs she may have while she is there for this training opportunity. Any additional costs such as activities or souvenirs she may want will be her financial responsibility. This option will ultimately save the clinic money because we will not have to incur the cost

Accomplishments: Completed six principal NIEHS Administrative Officer Efficient Spending travel data calls within specified deadlines for the Grants Management Branch linked to 10 employees and the Exposure, Response & Technology Branch connected to seven individuals. Actions required to complete the task consist of writing and proofreading statement of purposes directly related to the individual’s reason and motion for participation and attendance at the meetings, conferences, and workshops, scientific or general specific topic labeled event functions. Roughly, hundred and four purpose statements were generated and two hundred and sixty-three entries made into the NIEHS Conference Travel Tracking System in anticipation of employee’s attendance at various events and all involved collaboration with the traveler and conference host personnel. A purpose statement is required for all travel proposed regardless of actual approval by the Branch Chief or Program Director. Additional research was done that necessitate examination of the events websites, agendas, and traveler intel documents i.e., taxicab fare finders, x-rates (foreign conversion tables), and the General Services Administration (GSA) website was used to obtain City Pair Airfare Rates, and location per diem rates to prepare the

Bayview Center CHSLD is an English based long-term care that facility in the West Island on Bord-du-Lac that provides rehabilitative, restorative, and skilled nursing care for residents. The Center was created in 1956 by the Samatas family, presently it takes 128 residents in four different units, each unit having 32 residents. Every resident has their own room, where personalization is encouraged, it contains of a call bell, an adapted bed, a dresser, and a night table.

In the area of collegiate sports, there have been numerous heated debates about the integrity of many things concerning the NCAA and how it handles legal and ethical issues. Two well renowned scholars tackle this issue in their co-authored book entitled

Daren Stevens has owned and operated a golf driving range (Dog Leg Golf Course) for a number of years.
