You left me with the Supported Self Assessment form, I will fill in the first couple of pages and I am also sending you a report from Jill Carvassi, she is Shay's named nurse from the CAMHS team, it explains Shay's needs and how to address them, I think this information should be sufficient for now? If not I will be asking Jill to support me in filling future forms, I have a lot of paperwork to get through at present
UCM: CPSW met with Mr. Mauer for supervision at Brookdale and the visit went well so far. Mom and dad played with their son. Ms. Mauer changed Kaison's diaper and feed him during the meeting. Kaison seemed little tired today. Mr. Stark joined us today and he mentioned that he was in jail for two days and his case was dropped because it was wrong allegation from a friend. Ms. Mauer, Mr. Stark will be meeting their county nurse today after supervsion visit is done. Kaison is doing well and he had no fever and no coughing during the visit. The foster provider stated that since he got the shot for croup to help him breath better he has been doing well. No fever and no coughing so far according the foster provider. After enrollment into the program was complete, HARRIS nor MCGHEE presented HILL with any questions of how the program worked, timesheet approvals or submissions. HILL monitored any changes in DANIEL’s need for services through face to face monthly visits which lasted about 40 minutes. Changes in the need for services may be due to an admission to the hospital, a long-term nursing facility or relocation to another state.

CM spoke to Kelly (Case Manager) and Alexia (youth) via phone conference in regards to a follow-up on youth. CM was informed that youth was able to remove her braces and has been rebuilding her relationship with caregiver. Youth is willing to reside with caregiver after completing program. The team discussed after care services for youth; the team rescheduled CFT meeting for Wednesday, 8/16/17 at 12:00pm to discuss transition plan, in-home services and education needs.
I believe that the intake form reviewed all necessary questions needed to determine the client’s social and medical history to develop personalized goals for the client’s individualized treatment plan. I will begin to discuss the areas that the intake form reviews in order to obtain the necessary information to develop a treatment plan for the client.
Patrick Henry was a prominent lawyer, politician, and orator in colonial America, representing Hanover County, Virginia in the Second Virginia Convention and Virginia in the Second Continental Congress (history.org). He is most well known for his proposition to the convention concerning the institution of a standing army to defend Virginia, and his speech given in defense of said proposition wherein he boldly challenged the crown to “Give me liberty, or give me death” (historicstjohnschurch.org).
In Patrick henry’s speech at the second Virginia convention on march 23rd 1775 he powerfully assert that the British are about to change their way of life, and that they need to take steps to prevent them from destroying their newly founded country. Patrick Henry becomes a very unsuspecting leader in order to save his country from the impending doom of the British invasion. but in order to become a leader must one always risk life or limb.? or must they lead troops into battle? A leader should be able to know what is best for his people. Patrick henry never stepped foot on a battlefield, nor did he lead troops into battle. He was just an intellectual that saw a danger and he addressed it.

He says that there is no one more supportive of patriotism than he is, but acknowledges that there may be people with different opinions than his own opinions. He appeals to hi credibility and ethos in order to open the minds of his opponents to his position on the topic. 2.

These three speeches The Declaration Independence, Patrick Henry, and “From Subjects to Citizens”, had basically the same interpretation. In Patrick Henry's speech he mainly was trying to convince the delegates to secede from Britain; moreover, to fight back against them. The Declaration of Independence states that all men are created equal and are entitled to "life, liberty, and the pursuit of happiness." "From Subjects to Citizens", people are not always sure they have the power or resources they need to make decisions or to act. The government does not give everybody a chance to express their feeling about what's going on around them.
Patrick Henry, a devoted patriot and wise man fulfilled a position in the Virginia convention. After the Intolerable Acts imposed by King George on the colonies, Americans suffered an unfair rule, where Great Britain took control. In 1775 Patrick Henry introduced a resolution to the Virginia Convention to form the local militia to be prepared to fight the British. In order to gain approvals from his collies, Henry employs rhetorical appeals witch help him urge his purpose and take the lead with the use of an urgent tone witch induces his audience to support his statement towards war.
“Resolved, that these united colonies are, and of right ought to be, free and independent states,” stated in the Independence resolution. For over more than one decade, people from all over the world have striven to see the nation come as one, and rather not see it drift apart from each other and hate one another. Patrick Henry’s speech has incensed violence and fights the British which not everything should be about war; one way to fix these issues of revolution is by having more peace and love among nations and states. Opposing to the speech, that violence is never the answer; however, Patrick states, “-we must fight, pg.1.”
Freedom, the one thing that America is know for. Freedom doesn’t just happen, it is earned. Patrick Henry was the man who said the first words that needed to be heard to bring about the freedom the we have today. Patrick Henry was born May 29, 1736, he lived his life defending people who couldn't defend themselves and became a attorney. In 1776 and 1784 he became the first and the sixth governor of Virginia. He was married twice, the first was in 1754 to Sarah Shelton, who later passed away in 1775 to what was believed to be postpartum psychosis. He was then married to Dorothea Dandridge in 1777 until he died on June 6, 1799.

To begin with, back in the century when Abraham Lincoln had his presidency running in the United States of America the Emancipation Proclamation took place in September 22nd, 1862 were Abraham Lincoln declared the United States to become a free slavery state, later on Fort Wagner in July 18, 1863 a year after the Emancipation Proclamation of Abraham Lincoln, black American soldiers serving for the United States fought for the first time against the assault in Fort Wagner were one young brave American soldier named William Carney during the assault showed pride and honor for serving the United States military not only fighting for America but showing his true spirit for not giving up the American flag during the assault until the colors of the flag that represented America was returned back and only to the United States and United State soldiers that were serving for America, and finally the Summation of Gettysburg Address happened in November 19th, 1863 were Abraham Lincoln gave
