On 6/15/2015, Client attended to her ILP meeting with her oldest daughter Joeli, son Pablo and her granddaughter Anna. Client stated that her daughter Nurys was still at school.
PA/Entitlements/ Benefits: The client stated that she has an appointment at the PA office on 6/2/2015 at 2p.m. Client stated she went to her PA appointment but her case continue pending. Client was provide with a Food pantry list from around the shelter area to help herself to put food in the table for her family into her case change status.
Employment: Client is currently working on alternate staffing for one year and a half.

SOCIAL SUPPORT UPDATE: Client reported she wasn’t feeling well. She has bronchitis and takes prescribed medication. CM inquires if the client got in touch with her children and sister. Client replies “her children live in Florida and she doesn’t know the whereabouts of her sister. She also reported no community support.
Social Services: On 02/17/2017, Ms. Hair and her son Aaron met with the family assigned Case Manager for the family ILP Document Review. Ms. Hair is expected to meet with her assigned Case Manager bi-weekly. Ms. Hair’s next ILP appointment is on 02/02/2017. Case Manager asked Ms. Hair if she is aware that she is not in-compliance with the terms of her ILP, Ms. Hair stated yes. Case Manager asked Ms. Hair if is aware that failed to attend her ILP meetings since 01/23/2017, Ms. Hair stated that she was aware. Ms. Hair last ILP was conducted on 01/23/2017. CM was out on vacation from the period of 5/31/2017 to 6/14/2017. On 6/20/2017, CM met with the client to complete Bi-Weekly ILP Review. In the meeting client was satisfactorily groomed and dressed appropriately for the weather. Client affects and mood was inappropriate. Client continues to denied suicidal or homicidal ideation. Client reported she wasn’t feeling well and she was given “Bed Pass”. CM observed that the client sometime can appears to be manipulative and she will always find excuses for her behavior.
On 9/12/2015, CM met with the client to complete Bi-Weekly ILP Review. She was well groomed, and dressed with proper attire. She made eye contact appropriately. Her affect was appropriate. CM inquires if the client had any recent seizure disorder. Client replies “No” because she is not spending time in the shelter. She continue to relate during the day spend it with her children and grandbaby.
Ms. Cruz informed HS that client last permanent address was 245 Walnut Ave. Trenton, NY 08609. Client stated that they need to move out from the apartment because there was an altercation between the husband and another person who trying to kill him. Client came into social service after receiving a SUS appointment slip left at the front desk by CM 10/3/17 to meet on 10/5/17 at 4pm. Client stated she only had a few minutes to talk because she had to cook dinner. Client informed CM her PA account was closed and she replied once again. Client also reports she has to visit Connecticut, PA office for a letter stating her PA case is closed in that state. Client informed her CM all documents requested will be submitted within a weeks’ time. Client reports two of her four daughters Selah McKenzie and Olivia Lue have a doctor appointment tomorrow October, 5, 2017. Client and this CM was able to complete all three forms. Client stated to this CM working on Friday at 12PM to Monday at 12PM each week as home health aide in New Canaan for an 89-year-old women. Client expressed happiness to be employed. Client stated being homeless since June of 2016 when due to getting kicked out of father's household. Client stated needing help to get housed is the reason for requesting case management services. Client stated to this CM being kicked out of father's house was due to drug usages. Client is currently receiving $383 monthly from SNAP and make roughly $1890 monthly from job. Client stated to this CM utilizing the Day Street Clinic in Norwalk CT as a primary care provider. Client also stated to this CM receiving therapy from Rebecca at Day Street Clinic for pass history of trauma. Client stated to this CM being raped three years ago and have a history of sexual abuse. Client stated to this CM medical history contains heart murmur. Client currently is taking 500mg of trazadone nightly. Client is diagnosed with depression and bipolar disorder. Client stated to this CM having 8 children, 6 biological and 2 adopted. Client family history contains depression and high blood pressure. Documentation of what took place between the determination to cease the benefits on 12/28/16 and the determination to grant the benefits on 04/27/17 was not found in the electronic file. Perhaps, there were additional records that clarify the disability determinations that were not available at this time.
Smith in her home. Ms. Smith was in good spirits and her children were still asleep. She was informed that she needed to bring in a copy of her updated I.D. or Driver’s License. Ms. Smith agreed to have a copy of her I.D. prepared for the next HV. She was asked by the Housing Specialist if she had any goals that she was ready to work on. Ms. Smith stated that she was ready to take her GED test, but wasn’t able to pay for the test due to having limited funds. The client was told that the Case Manger would find out if the agency had funds to pay for her GED test. After the discussion the client was informed about the Home Visit form that needs to be signed by her at the end of the visit for PSH (Permanent Supportive Housing).
