CC Mrs. Estell is a 47-year-old female here today for followup and discussion of recent events in her life. The patient has not been seen in the office since December 2014. HPI The patient tells me she is under a lot of stress of late. She has suffered a fracture of her foot in the past year. In addition, she had undergone the surgery for her back with Dr. Banco and tells me that she is still recovering from that. She has been going to physical therapy, which has been helpful. She is working with Dr. Greenspan for her pain medications and they are slowly starting to wean down. She is hoping with her next visit that they can all be stopped. This is improving status post her surgery with Dr. Banco down in Boston. She is continuing with physical therapy, as well as her pain management with Dr. Greenspan and feels that she is definitely making progress with that, which I am quite pleased about. I have encouraged her to continue to work on those exercises and I will follow along. Depression and anxiety. She is being followed by Dr. Lidstrom. She is really struggling of late since the passing of her father-in-law and dealing with her mother-in-law and the emotions surrounding this change in their life and I am pleased that she is going to counseling as I think that would be very useful for her. She will continue her follow up with them and continue with her current medications. Migraine headache. She is seeing Dr. Somireddy. She is going to go ahead and continue her follow up with her as well and will continue with her current
Ms. Sunshine recently had a myocardial infarction, with 90% of her one main artery occluded. She is a 54-year-old African American female who decided to travel from Denver to Boston to visit her son and new grandson. Prior to leaving Denver, Ms. Sunshine has been experiencing chest discomfort, dyspnea, fatigue, anxiety, and flu like symptoms. Her son Mitchell noticed his mother was in distress, he called for an ambulance and Ms. Sunshine was immediately taken to the local emergency room department.
Since I have last seen the patient, she tells me she has been to see [Mary Bluen at Orchard Park in Portsmouth.] She has been seeing her in counseling now for the last six to seven weeks and her next appointment is tomorrow. She says she has been struggling lately with her feelings. She established a relationship with a superior at work who is married and she herself is married. She says this is something that she is trying to work through to figure out why she is making these choices. Her plan is to ultimately end her marriage. She is not sure if she will continue to see this male at work again or not. She is just not sure what her choices are. She said on top of all of this, she just found out today that she has been given a 90 day noticed, so she is not sure if she will even be able to keep the same job and she is fairly certain that this is follow
A review of her medical records indicates that she was recently discharged from home health and from physical therapy.
The government, local authority and independent organisations are making every effort to safeguard vulnerable adults, children and young people by introducing new policies and procedures in workplace. With no exemption in care home setting, company policies and procedures are reviewed and updated regularly as a result of the change of laws, regulations and quite commonly from recommendations after serious case reviews and inquiries particularly. There are few serious case reviews and inquiries have had impact on the change of policies and procedure in recruitment process.
At today's visit, she is accompanied by her daughter. She is found lying in bed in a fetal position. She is awake and alert. She complains of numbing, tingling, shoot pain in her legs and feet which she contributes to her neuropathy. She complains of chronic back and shoulder pain which she describes as dull. She rates her pain as an 8/10 in severity. She states that her pain does not radiate but is worse when she moves or changes position. She
RC is a female of unknown age presented with a chief complaint of a persistent headache. Specifically, RC describes the pain as being located on her skull’s bony ride behind her left ear. The pain is localized to that region, does not travel or radiate to any other areas, and is described by the patient to be felt on her skull bone. The pain is described as being sharp and not dull. RC began noticing the onset of the pain about two months ago and reports it to have gradually gotten worse in severity. The pain gets worse towards the end of the days. The sharp pain makes her feel some “pressure” and “fogginess” in her head as the pain gets worse. Patient does not report any associated symptoms.
At today’s visit she is found sitting in her wheelchair. She is awake, alert and confused. She denies pain and shortness of breath. The facility staff reports that the patient has decline functionally over the last month, with a very poor appetite. The facility nurse reports that Psych recently adjusted the patient’s psych medications and the patient appetite and affect has improved. The nurse reports that the patient often gets anxious and tries to hit the staffs. The patient

When looking at Case 2.1 in the casebook, it is stated that the young boy treats other students’ aggressively. A social worker should intervene in order to look out for the best interests in the other students. Another example of when a social workers’ obligation to the broader society would override a clients’ rights or interests would be if a client is planning to harm him or herself. Casey’s parents both work full time and report having little free time. Training for PECs for the first phase is only about 2 days, (PCE site) much shorter than sign so they might consider the training. Presently they have to strain to hear her handful of word approximations or be led to what she wants, or needs. Her frustration and disgruntlement are due in part to not being able to make her needs known, which often results in meltdowns. Dealing with her behaviours must be time consuming for Casey’s parents and others (relatives, support worker, and etc.).
Henry is a 21 years old collge student, he is an international student from China, he was born in China and live with his parents until he was 17 years old.Henry is now living in a shared house. Henry reported that he did not have any health problem in his childhood. Recently Henry feels he has been really depressed and he has trouble fall asleep at night. Because he feels he can not do well at school also his parents doesn’t understand him, he feels he can’t focus in class and easily got distracted when doing homework. Henry stated he always doing many tasks at the same time, but normally can’t finish them. And Henry also stated that he have difficuties when communicating with others, and he thinks is hard for him to keep listening
On a hailstorm Saturday night, my manager called and asked me if I could train the new girl on her first shift from twelve a.m to eight a.m. As I was leaving my house, I saw the wind hugging the trees aggressively. The rain was so intense as it poured down endlessly. Entering the office of The Parking Spot, Zarah was hyperventilating because she didn’t know how to checkout a customer who was complaining. In the register area, I explained to Zarah how to provide customer service and how to check them out accordingly: enter their ticket in the validation machine, apply the discounts, and insert method of payment. Then, a red hair woman was furious about Zarah not knowing how to do her job. She told Zarah and I that we shouldn’t work
There were several helping skills used in my case study that allowed me to properly assess and treat John and his family. The first helping skill I used was the family systems. As a human service practitioner, systems theory, permits me to take into account the entire “system” with which an individual interacts. This system allowed me to look at the entire dynamic of John’s case. John interacts on a daily basis with his parents, younger brother, new-born sister, teachers, and classmates.
I sat and watched a forty-year old woman who could not even pick up her own tissue as she sobbed about her condition. She had lost her hands and legs to no fault of her own. I was mesmerized as I watched my father skillfully interview her with compassion while he was obtaining the information to evaluate the legal basis of her claim. As a child, I would often accompany my father to his office, the courthouse, other law firms, and clients’ businesses, which only increased my curiosity for all of the work behind investigating and developing complex cases. From a fairly young age, I enjoyed making copies, sorting folders, and making binders that went down the hall and even went into a plane for a deposition. I would especially look forward to the days when my father would go to the Harris County Civil Courthouse to try cases, as well as for depositions and hearings. Additionally, I was very intrigued by the entire process of building up a case from scratch and getting it ready for settlement or for trial. Before long, I was able to interview and obtain a job as a teenager where I was able to speak
On April 18, 2016, I attended my assigned clinical site at Maimonides hospital. My assignment consisted of taking care of a 95-year-old female, who I will name as Ms. Lily due to confidentiality. Ms. Lily was admitted on April 9, 2016 for shortness of breath. She is alert and orientated to person, place, and time. She also has a history of atrial fibrillation, diabetes, HTN, and high cholesterol. Ms. Lily was also scheduled to have coronary angioplasty on April 18th.
She finds it difficult to continue on with daily activities and will often have to call in sick at work or cancel plans and spend the day in bed (she has called in sick 4 times over the past 3 months because of this pain)
