Providing the best care to each patient starts with providing the proper amount of staff members to each unit. Looking at the needs of different units not only allows administration to see areas for improvement, but also areas that are being handled correctly. Utilizing the indicators provided by The Joint Commission, 4 East, a pediatric medical/surgical floor, has a high rate of falls and nosocomial pressure ulcers that appears to be related to the increase overtime nurses have been working for that floor (Nightingale, 2010). Research has shown increases in adverse events have been related to nurses working over 40 hours a week (Bae, 2012).
Theresa Magana Ms. Nauls-Jones AP English 3 26 November 2015 A Rhetorical Analysis of Jonathan Edwards’s Sinners in the Hands of an Angry God, Widely regarded as “One of America’s most important and original philosophical theologians”, Jonathan Edwards, in his sermon, Sinners in the Hands of an Angry God, provides a glimpse into the theology of the great awakening. Edwards’s purpose is to awaken audiences to the horrific reality that he believed awaited them should they continue life without devotion to Christ. He adopts an ominous tone with a sense of urgency in order to spread fear in the minds of puritans so that they may repent for their sins and turn to God before it is too late for them. Edwards opens his sermon to the puritans by stating, “So that thus it is that natural men are held in the hand of God; over the pit of hell; they have deserved the fiery pit and are already sentenced to it.”
Dr. Linda Aiken is the leading researcher in nursing staffing rates within the United States (Kerfoot & Douglas, 2013). She suggests that each state follows California’s example of conducting research to determine a minimum staffing ratio and making it a law (Kerfoot & Douglas, 2013). The California legislation enhanced nurse staffing in hospitals across the state and improved the patient care results of millions of patients (Kerfoot & Douglas, 2013). After the bill went into effect, job satisfaction increased, and the nursing shortage ended (Kerfoot & Douglas, 2013).
In order to make the best decision for the staffing of both locations it is essential to 1st understand the requirements required at the locations. To do this the current job functions must be analyzed, and documented for all positions. After documentation is done it the job duties of each position will be reviewed. At this point I can decide if the current job functions meet the requirements for the location, or if new duties need to be added. The quality of care is also influenced by the health care organization or system, which involve not only staffing levels, but also patient need, the availability and organization of other staff and support services, and the climate and culture created by leaders in that setting. The same nurse may provide care of differing quality to patients with similar needs under variable staffing conditions and in different work environments. Mandatory staffing ratios have been suggested as a way to meet nursing staffs’ concerns of high nurse to patient ratios. Mandatory staffing ratios are used as a way to reduce workload and patient mortality and are aimed at addressing the perceived imbalance between patient needs and nursing resources. (American Nursing Association, 2014). However, issues have been raised on applicability of staffing ratios since it could lead to increased costs without the guarantee of improvement in the quality of health care and could also lead to unintended consequences including unit closures, limited infrastructural development and limited access by patients (American Nursing Association, 2014).

The American Nurses Association supports a legislative model in which nurses are encouraged to create staffing plans specific to each unit. This approach will aide in establishing staffing levels that are flexible and can be changed based on the patients needs, number of admissions to the unit, discharges and transfers during each shift (“Nurse staffing plans,” 2013). This model will assist in keeping the unit staffed appropriately and organized in need of a change during each shift. Without an organized plan like this, a nurse may be required to take on a new admission and already have too big of a workload.
In recent years, the healthcare industry has seen a significant decline in the quality of patient care it provides. The nurse manager has to explain to the management of the benefits of change in providing adequate staffing all the time. Adequate staffing helps staff retention. Staff retention saves a lot of money in terms of orienting new people to the unit. Safe staffing always helps in the reduction of falls, infection rates, pressure ulcers, decrease hospital stays and death. Flexible and creative scheduling is essential for retaining staff and promoting a positive work climate (Grohar-Murray & Langan, 2011). Adequate staffing with good staffing ratio will help nurses to concentrate on their patient care which may help in a reduction in medical errors and lawsuits to the hospital.
As an early childhood educator, in an effort to support families who have a lack of extended family support, I will make sure to provide them with resources that support their family needs. Resources that are based in the school setting could be childcare, tutoring or expanded learning programs in the afternoons. These school based resources can help the development of their children and offer parents more support. Other resources are more community based, such as parenting classes, enrichment programs and many more.
