On December 26, 2015 at approximately 2329hrs, I received a call for service to respond to 231 E. Clark St. in reference to a suicidal subject. Stg. Henke and I made contact with a white female claiming to be the mother of the suicidal subject outside of the residence. Stg. Henke and I made contact with a white male at the front door inside in an enclosed porch of the residence. I observed the male’s hands and blue jeans he had on covered in blood. Sgt. Henke and I advised the male we received a report of him trying to hurt himself through via text message to his wife. The male advised he didn’t send any text messages stating he was going to hurt or kill himself. Sgt. Henke and I asked the male what his name is and the male refused to identify
At approximately 7:27 a.m., Detective J. Figueroa arrived Cape Fear Valley Medical Center 1638 Owen Drive Fayetteville, North Carolina 28304 in-reference to an additional victim suffering from a gunshot wound. The individual was identified as Kenneth Alexander Shaw (black male, 7/5/1988) who arrived in a gray Ford Fusion North Carolina Registration PAB5727. Detective J. Figueroa was contacted by Officer Catlett who advised an Amos rashaad Shaw (black male, 10/10/1989) was in the parking lot and had brought his brother Kenneth to the Hospital.
On 12-05-17 at approximately 1017 hours, I responded to 442 N. Los Robles Avenue regarding a psychological evaluation. PPD Dispatch advised that a female, White was yelling incoherently at the above location. Officer Gawlik #3116, Officer Herrera #3595 and Officer Killgore #7231 were on scene prior to my arrival.
On 08/07/15 at approximately 1820 hours, my partner (Ofcr. Vanlenzuela No.38161) and I (Ofcr. Nunez No.36181) were working North Hollywood patrol, assigned to unit 15A84 W5. At that time, we responded 10849 Fruitland Dr for an “Attempt 211 Suspect.” Comments of the call indicated the P/R was the wife of the victim Melvin Salinas. Suspect was described as a male black “transient” armed with a knife tried to take a vehicle from the parking garage Inc# 150807003722.
On 10/11/15 at approximately 1944 ICS was activated to an offender OID 236740 who was bleeding and Medical was requested to respond. I do not think that the OT should belittle the OTA as the OTA was probably just afraid they would hurt the child. I do believe that the occupational therapy assistant should have asked for assistance earlier in the therapy if she/he did have concerns. The OT should demonstrate beneficence to the OTA and the patient as it is still a need to be addressed. The OT should explain to the assistant the need of laying prone to and why it is so important to the child. The child may not have the best of insurance and could be on a limited supply of therapy benefits. They could also be so behind in their development just because the assistant was uncomfortable with the tube. This demonstrates justice as it will benefit the child to get the treatment

The patient returned to the clinic after his admission to Johnson Memorial Hospital twice. The first admission was from 10/31-11/3/2017 to address his major depressive disorder and substance abuse. It was recommended for the patient to seek a high level of care-outpatient psychiatric services, PHP/IOP. Then the patient was readmitted on 11/4/2017-11/06/2017 due to a bicycle accident and alcohol consumption on 15 shots. Its also documented that part of his admission was for suicidal ideation of which the patient denied having suicidal ideation during the second admission, but admits to having sucidial ideation during the 1st admission. The patient is scheduled to attend an appoinment through CHR tomorrow at 8:30am to complete an assessment
At 1:30 pm, 9/3/17, dispatch received a 999 call regarding concerns about an individual male at the address of John Foster room 1.57, Liverpool. The caller identified himself as Mark, He rang because he was worried about his ex-partner, a male individual. He advised police handlers that due a recent break up with his ex-partner the male individual had been rather sad, Mark has attempted to contact his ex-partner but despite attempts Mark has had no contact with the male individual.
Note: Emergency Medical Services (EMS) was requested by the reporting party and Lt. G. Flores #110 would be checking on the individual until I arrived.
FPTP is the second most used electoral system worldwide, and in the UK it is used primarily in general elections to reach a fully representative, democratic electoral system. In assessing whether we should scrap FPTP, we must first establish what an electoral system ought to provide. The purpose of an electoral system in the UK is to fairly elect representatives for the population to make up an effective government, as we operate with a system of representative democracy. For an ideal democracy to function, an electoral system that delivers a strong constituency link, genuine voter choice, an effective government is fundamental. It is also important for a system to operate simply and with transparency for the population. As Tom Stoppard once wrote, “It’s not the voting that’s democracy; it’s the counting.”
Mrs. Delappe is a 61 year old female who presented to the ED with suicidal ideation with a plan to overdose. At the time of the assessment Mrs. Delappe denies suicidal ideation, homicidal ideation, and symptoms of psychosis. She reports relational issues with her mother yesterday and becoming overwhelmed. She reports fleeting suicidal ideation for several years. Mrs. Delappe reports a history of attempt 30 years ago. She reports a history of anxiety and depression. She reports when she gets around her mother it only takes 5 minutes before she gets anxious. Husband was present at the time of assessment. He confirms his wife does have fleeting suicidal ideation from time to time, however has never acted on them.
Upon arrival, I heard inaudible screaming from the rear of the residence, at this time through the open rear window I observed a male subject screaming from the staircase. I made contact with the male subject inside his residence (Diaz, Johnny DOB 08/01/72), Diaz appeared to be under the influence of alcohol. I detected an odor of an alcoholic beverage emanating from Diaz. While speaking with Diaz, I observed a cut on his left middle knuckle. Diaz related that he injured his hand working on trucks. I observed an open syringe on the kitchen floor laying next to a burnt metal tin (one photo taken and later placed into inventory). Diaz related that he is a heroin user and is taking prescribed methadone.

Statistical data yields information about adolescents at a rate of 21% for suicide ideation and 14% for suicide attempts. In their study A New Perspective on the Anti-Suicide Effects with Ketamine Treatment, Lee, Syeda, Maruschak, Cha, Mansur, Wium-Anderson, Woldeyohannes, Rosenblat, and McIntyre, propose that the intravenous administration of Ketamine for patients suffering from acute suicidal ideation, is an effective treatment protocol. As described in the Journal of Clinical Psychopharmacology, this possible stopgap for an otherwise option vacant medical emergency may be a viable choice for patients experiencing acute suicidal ideation to include patients with a history of treatment resistant depression. Someone diagnosed with treatment resistant depression or treatment refractory depression can be defined as one who does not respond to pharmacological treatment
