In the past few years there has been increasing discussion about how to provide adequate care for the increasing number of veterasn who are eligible for care through the Veterans’ healthcare administration (VHA). There are concerns is that the VHA is not providing the level of access, efficiency, and quality of care that veterans expect. Lee & Begley, (2016) suggest access to care for the veteran population may be resulting in poor health outcomes. In response to these concerns, the Veterans ' Access to Care through Choice, Accountability, and Transparency Act (VACAA) of 2014, also known as the Veterans Choice Act, was created to improve Veterans’ healthcare. The VACAA proposed to do this by expanding the number of options veterans have for receiving healthcare, by providing access for healthcare at non-VA care centers as well as providing for an increase in staffing at VA facilities (U. S. Department of Veterans Affairs, 2016).
In the United States, soldiers are currently returning home from war broken and scarred. While some physical wounds are clearly visible others are often hidden/invisible to the naked eye, but men and women proudly wear these wounds as a badge of courage and honor for protecting our nation. These men and women come home rightfully expecting help, assistance and care and get hit with the reality of poor care from the Veteran Health Administration and Department of Defense.
Jones is a 64 year old well nourished man with a history of long-standing non-insulin dependent diabetes mellitus (NIDDM). He had an open heart -surgery bypass graft 7 weeks ago. The graft site got infected and had to undergo an emergency surgery to improve circulation to his left lower limb. Mr. Jones is relieved that his leg was saved and he is now being prepared for discharge. He wants to regain his strength so that he can start doing the things he loves. Currently Mr. Jones needs moderate assistance depending on his level of pain or fatigue and ambulates with a walker. Mr. Jones still needs help with activities of daily living. He has urinary and bowel control problems as side effects from multiple antibiotics. Mr. Jones is married and has 4 grown children that provide him with love, care, and support. The nursing staff is teaching Mrs. Jones how to perform sterile wound care for her husband as he will need dressing changes three times a day, blood glucose monitoring and a healthy diet. Social service is helping to keep communication channels open between Mr. Jones, his family and the nursing staff to maintain psychological and emotional health.

Nearly ten years ago I entered the Army service. Through these years my work in the medical service as a medical laboratory technician, biomedical research, and most recently as a registered nurse has challenged me to examine my own practice and technical skills. I have learned about compassion, respect, empathy, equity, understanding, benevolence, and other attributes that contribute to being a proficient nurse. Our service members, families, and military culture present unique challenges that demand diversification, flexibility, and adaptation to sometimes overwhelming circumstances. My love for our military men and women and the families that stand behind them causes me to seek best practices, and innovation. The military has done a tremendous job at advancing battlefield trauma care, and statistics indicate that we have standard setting rates of battlefield lifesaving.
Some women may even prefer not to report any allegations of being pressured sexually for fear of what might happen to them if they do. Although it is not lawful to retaliate against an individual for exposing employment practices that discriminate many people would seek retaliatory efforts. Therefore there are many women who feel it may be better for them to suffer in silence than to deal with revengeful or vindictive tactics such as being denied promotions or being scorned by other women who may think that the victim is pressing charges for attention. Because of the nature of such unprofessional behavior the victim may find him or herself very alone and no one wants to go through that especially after suffering through the harassment itself.
