Telemedicine is a vast subject, but as yet there are limited data on the clinical effectiveness and cost-effectiveness of most telemedicine applications. As a result, objective information about the benefits and drawbacks of telemedicine is limited. The purpose of this paper is to discuss how technology has impacted the health care delivery system. Health care technology is advancing at an increasingly rapid rate in the United States, and globally. Patients and providers are witnessing these changes through the use of telehealth, and telemedicine applications. Telehealth, and telemedicine are often used interchangeably, however there is a difference between the two applications. Telehealth is the use of technology to deliver health care, health information, or health education at a distance (U.S. Department of Health and Human Services, n.d.).
Telehealth Introduction Telehealth is the use of technology for the exchange of health information in a two way communication from one location to another using a variety of tools or applications. Tools and applications such as smart phone devices, video conferencing, emails, and other wireless applications are used for the convenience and improvement of delivery of healthcare. It has the advantage of connecting healthcare personnel and primary care provider with patients and provides knowledge through the network system regarding patient care and monitoring (Katzman, 2013). Telehealth is a cost effective way for the Advanced Nurse Practitioners (ANP) to have the ability to reach a wider geographical area and broader populations.
To evaluate and study the impact of telemedicine in health, a systematic literature review was conducted. An electronic research through the University of Maryland library was done the articles selected dated from 2000 to 2014. A total of 12 articles was reviewed that dealt with clinical outcomes, efficacy, patient and provider satisfaction. Telemedicine as an emerging field can greatly improve the outcomes of healthcare thus resulting in decrease the delivery cost of healthcare. While the results of the studies are inconclusive on some of the areas, there are some strong supports for patient satisfaction. The literature reviews cost effectiveness, provider satisfaction, patient satisfaction and clinical outcomes.

Telemedicine is similar to Telehealth. Telehealth refers to a broader scope of remote healthcare service but Telemedicine refers specifically to remote clinical services. Telehealth can also refer to remote non-clinical services. For example provider training and meetings (link 3). In the past the network is not as fast as now, thus telemedicine is only use in clinical services. Now, due to the development of network, more information can be transferred in a shorter period, thus Telemedicine can be to provide other non-clinical services. The word “Telehealth” refers to both clinical and non-clinical services.
According to Breslow et al. (2013) telemedicine can be broadly defined as the use of telecommunications technologies to provide medical information and services. Telemedicine includes medical uses of the telephone, facsimile, distance education, teleconferencing to image-sharing to remote patient monitoring, and provide clinical services to patients (Breslow et al., 2013). Telemedicine is increasingly being used as shorthand for remote electronic clinical consultation.
Many of these barriers exist from lack of essentially knowing the benefits of what the incorporation of telehealth can offer within the aspect of this type of community. At times, there has been a great interest in telehealth surrounding its potential to reduce health care costs along with the improvement of patient outcomes, but at times there has been no known scientific evidence base underpinning it (McLean, Sheikh, Cresswell, Nurmmatov, Mukherjee, Hemmi & Pagilari, 2013).
