Allergies: Per mother, child is allergic to cats which cause him to “be itchy and have a stuffy nose.”

Kirkland gummy vitamin 1 per day
Developmental History: Demonstrated development/behavioral milestones. Ht and wt no periods of rapid gains or loss. First tooth erupted at 5 months currently has about 20 teeth. Started walking at 10 months. Knows his name and address, plays, climbs, skips, jumps and tumbles. Dresses and undresses without assistance. Can count to 20, knows his basic colors, understands basic concepts of time. Able to speak in sentences, tells stories and conveys needs. Enjoys playing, dancing, singing and acting with neighbor’s two children. Is toilet trained and doing well up until the past 2 days. Terms for toileting are pee pee and boo boo.
Nutritional History: At birth was breast-fed on demand about every 3-4 hours. Satisfied after feedings. When mother not home, mother’s milk was frozen and thawed before bedtime. Breastfed until 2 years of age when weaning began without difficulty. Commercially prepared solid…
