EXECUTIVE SUMMARY FOR THE BHOPAL TRAGEDY HISTORY AND BACKGROUND Bhopal is the capital city of Madhya Pradesh, the largest and one of the most economically depressed states in the nation. At 1970s, the public health infrastructure, sewage and waste system in Bhopal are still in very poor condition . Besides, mass casualty emergency response system also lacking in Bhopal. Union Carbide Corporation is one of the largest chemical and polymer companies in the united state. In 1984, Union Carbide operated in 37 host country as well as in its home country, ranking 35th in size among United State . The main cause of the incident was the late transfer of the MIC from the Tank 610 which was use to produce pesticide to the SEVIN unit when they realized that water had entered the storage tank. During the time range from 12.15 am to 12.30am on 3rd December 1984, the investigation team had concluded that there was a transfer of MIC from Tank 610 although the data in the logs had been falsified. The team got an attempt to remove water from the Tank 610 when they knew that the safety valves of the system lifted and the pressure in the storage tank was high enough to release the MIC-water mixture compound. The transfer seemed to be impracticable as the remove of the one ton of the material was not enough to relief the pressure in the tank. The only thing that the workers could do was to remove the water that found in the tank. Anyway, the transfer of the MIC compound was too late because the pressure of the Tank 610 was too high to get control. Perhaps, this poor incident happened due to the technological, organization and human errors. The safety and maintenance procedures for the manufacturing plants had been ignored for months due to the financial problem faced by the company. Firstly, the MIC compound stored in the Tank 610 had overloaded. In reality, the content of MIC in the tank cannot be more than 50%.
