National Service Frameworks and National Institute for Health and Clinical Excellence guidance are integral to this standards based system, whilst The Healthcare Commission has an ongoing and major function to play in the assessment and review of all healthcare organisations.

The NSF and NICE create a means by which NHS trusts ensure the provision of quality standards by making NHS employees accountable for setting, maintaining and monitoring standards of care (DoH 1997). The National Institute for clinical excellence was founded in 1999 and consists of a number of specialized organisations: the NHS centre for reviews and dissemination, national prescribing agency, medical devices agency and institutes of public health. All aimed at creating and maintaining national standards through effective management and cost effectiveness, through audits and reviews of health policies. The commission for health improvement (CHI) aims to monitor the delivery of these standards provided by NICE and NSF through national surveys of the patients experience (Freedom D, 2002). This commission (CHI) sets out to review all NHS trusts including community care. Each NHS trust will be visited over three to four years and be reviewed to decide whether or not national standards are being met and NICE guidelines are being adhered to.

The definition of high quality care proposed by Lord Darzi in High Quality Care for All (2008) has now become enshrined in the Health and Social Care Act 2012. To achieve high quality care all 3 elements of patient experience, patient safety and clinical effectiveness must be present.

Home environment have to be safe for each service user and we need to make sure that it doesn't pose any harm or danger. Environment and feeling of the home is very important for our service users. Decoration and changes should be consulted with service users and it is their home. Each service user should be able to choose furniture, colour of the walls, carpet and other items for their own bedroom. The necessity for quality and safety improvement initiatives permeates health care. Quality health care is defined as “the degree to which health services for individuals and populations increase the likelihood of desired health outcomes and are consistent with current professional knowledge” (Kohn, Corrigan & Donaldson, 2000). Most care deficits are based in systems and processes and not individuals. Healthcare is has variation and complexity.

Health and Social Care Act 2008.- This Act established the Care Quality Commission (CQC), whose remit is to protect and promote the right of people using health and social care services in England to quality care and to regulate its provision.

In July 1998 the strategy for reorganising and modernising the NHS A First Class Service – Quality in the New NHS (Great Britain. Department of Health, 1998) was published. Its aim was to modernise the NHS ensuring delivery of high quality services for all. Quality was to be maintained by setting standards through National Service Frameworks and the forming of National Institute of Clinical Excellence (NICE). These standards were to be monitored by the creation of the Commission for Health Improvement. This was to be an independent statutory body which could scrutinise quality improvements at a local level and address any serious issues identified. Spot checks can be carried out and clinical quality information is made public. Additionally there was also the introduction of an annual national survey of patient and user experience of the NHS. It stated that quality of service should not be dependent on geographic location

The care quality commission is also a regulator which is independent for the quality of both health and social care in the UK. Similar to OFSTED, they examine and inspect as well as register health provisions such as hospitals, care homes, GP surgeries, healthcare practices, dental practices etc. The CQC have the right to give warnings, close down the practice, give a penalty notice, to suspend registration or prosecute the provider if they feel as though the services are not meeting the standards of quality and safety.
