Introduction
The nursing world is made up of complex systems with its numerous moving parts consisting of nurses, interdisciplinary members, supporting staff, technologies, and structures. When there are interactions between two or more individuals and systems, conflicts arise (Grossman & Velia, 2012).With emerging conflicts, theses systems can go into a state of disequilibrium. Change, good or bad is bound to occur. Change forces development of leadership in which nurses with their wide-ranging roles, are the most likely candidate. Therefore, nurses have an inherent role as change agents. It is a requisite in order to establish a health care environment that progressively upholds the nursing profession and aims to elevate quality patient care. It is only appropriate for nurses to acquire an awareness of change theories, the role of nurses as change agents, conflict theories, and conflict management. Ultimately, nurses must possess a steadfast commitment for the leadership role to achieve positive transformations in the nursing industry and society as a whole. Change Theories The Complexity Theory or Chaos Theory, for instance, explains disorder within an established structure (Grossman & Valiga, 2012). It requires nurses to be mindful of the uncertainties in the system and be adaptable. Nurses are to welcome changes and be equipped with innovation and creativity. Kristen Swanson’s relationship-based caring theory encompasses maintaining belief, knowing, being with, doing for, and enabling. Nursing is a profession with vast opportunities for growth and development. Each nurse has his or her individual passions; mine reside within obstetrics, women’s health, and nursing leadership. Nurse leaders play an integral role in the success of healthcare organizations. Nurse leaders shape the roles of nurses within their organizations. Nurse leaders seek methods to improve patient care. They also use innovation to gain efficiencies in care delivery and decrease healthcare cost. Many nurse leaders have an ultimate goal to aspire to be a chief nursing officer in a healthcare
Implementing a change in practice within these environments can produce anxiety or fear of failure in nurses, leading to a resistance to change. Several studies (Bozak, 2003; Lehman, 2008; Spetz, Burgess & Phibbs, 2012) expounded the need for a concise plan and clear communication between nurses and management when implementing a change of this nature. The use of Lewin’s Change Management theory can support nurses through the transitions and identify areas of strengths and resistances prior to implementing change. Without a framework for guidance, it can be difficult to keep on track.
According to American Nurses Association nursing can be defined as “the protection, promotion, and optimization of health and abilities, prevention of illness”. (American Nurses Association 2016) Nursing is a career where a person, should have compassion, a caring heart, and be devoted to their client care. Sometime person goes into the nursing field for the money, job stability, and because a person 's family member wants them to go into the nursing field. I feel like the future of nursing have yet to be determined.
The IOM particularly points out the need for strong and capable leadership of nurses. The report says that the professional nurses must produce leaders at every level of the system and accept primary leadership positions in politics and organizations. The implementation of health care reformation expects that the nurses must exhibit leadership capacity and high-level collaborative skills in formulation of policies as well as in nursing practices. The nurses should prove their potentialities for contemporary, advancing high-quality patient care at every stage. IOM says that acquiring leadership qualities from the beginning to the end of the nursing profession is very crucial to achieve the affordable health care to everyone in the challenging socio economic situation. The report clearly calls each nurse to take responsibility to attain leadership qualities to plan, implement and be a mentor of the society. My practicum setting will be an associate degree nursing program, Joliet Junior College (JJC), one of the oldest community colleges in Illinois. The mission of the school is focused on offering students an affordable, accessible valued education for a lifelong profession in nursing, and to serve the community (Joliet Junior College [JJC], n. d.). Accordingly, the school’s philosophy is based on preparing students to be exceptional providers in the growing healthcare system, providing safe and high quality care (JJC, n. d.). The foundation of the JJC nursing program is based on four major principles: client, environment, health and nursing (JJC, n.d), while their conceptual framework is grounded in the fundamental structure of three major theories which forms the basis of the concepts, skills, and principles of the nursing practice (JJC, n. d.). The three major theories are: Maslow’s Hierarchy of Needs, Levels of Prevention and the Nursing Process (JJC, n.d.). Joliet Junior College nursing school offers a competitive program and combines the traditional classroom experiences, in-class high-fidelity simulations labs, online augmentation theory lectures, and clinical experiences in health facilities throughout the community (Joliet Junior College [JJC], 2016).
Every day, a set team of nurses and nursing managers set out to ensure the health and well-being of their patients. To achieve this goal, a nurse manager must adhere to a specific style of nursing leadership. There are many different styles of leadership in the healthcare field. Bass and Barnes (1985) stated that the two most common are transformational and transactional (as cited in Frankel, 2008, p.24). This paper will define leadership, the two different styles, how each are executed, as well as pros and cons of each.
The third barrier discussed in the IOM’s Future of Nursing report was that nurses need to be full partners, with all other health care professionals, in order to redesign health care in the United States (IOM, 2010). To do this nursing education must add leadership aspects to the education programs. Nursing career is one of the few fastest growing fields in the health care industry not only in the United States but also in the world. Nurse practice has drastically changed in the last decade and as a result the need for changes in nursing practice is becoming more and more important. The Institute of Medicine report discusses so many aspects in nursing but this paper requires detailing the impacts on Nursing Practice (Transforming Practice), Nursing Education (Transforming Education), and the Nurse’s role as a Leader (Transforming Leadership). These three key aspects are discussed in the following pages.

Nurse leaders are faced with issues or problems on a daily basis that are often expected and sometimes unexpected. It “comes with the territory” so to speak. If there were no issues to solve or hurdles to overcome the necessity of designating a leader would not seem quite as important. Nurse leaders can tackle issues that occur in their work environment using nursing theory to guide them. Nursing theory provides a framework that nurse leaders can use to implement interventions or changes to positively impact the staff they lead. This framework of the theory will set the standards for achieving the desired outcomes and is based on knowledge that is gleaned from practice and/or research.
Being a registered nurse affords one the option of working in many diverse healthcare settings. In any practice setting the climate of health care change is evident. There are diverse entities involved in the implementation and recommendation of these practice changes. These are led by the Robert Wood Johnson Foundation (RWJF), the Institute of Medicine (IOM), nursing campaign for action initiatives, as well as individual state-based action coalitions. Nurses need to be prepared and cognizant of the transformations occurring in health care settings as well as the plans that put them at the forefront of the future.
In the healthcare field, nursing leaders and managers face consistent issues in their respective practices that force them to alter the way they work and the way they think. In taking on a role as a leader within the field, nursing leaders and managers also take on the role of ensuring that work within an organization runs smoothly regardless of new issues that may arise in the healthcare arena. For instance, in today's healthcare environment, the issues of nurse shortage and nurse turnover have the capacity to alter the healthcare field and many of its respective branches and organizations should these problems not be managed properly by the leaders in the field. In viewing the issue at hand and in discovering how nursing leaders and managers are expected to act, and do act, in order to approach this issues, along with pinpointing the best approach possible to aid this issue, one can better understand which leadership styles are necessary for leaders to function.
Without competent leadership change can have negative impact on both the employees and the patients. According to Wright (2010), lack of leadership in change management could lead to adverse outcomes such as resentment, rebellious attitudes, and could end up damaging the same people it intended to make better (p.20). Therefore, experts warned that it is very important to have competent leadership at the helm of change management (p.20). As the focus of providing leadership shifts to nurse administrators who are often in the frontline of change management, nurse administrator should use essential tools such as the American Organization of Nurse Executives (AONE) five domain of competence as a framework in change management.
According to Sieloff and Raph (2011), nursing management theories are used to describe, explain, predict or prescribe nursing acts. There are a variety of ways to solve a nursing problem or issue, but nursing management theories are specifically designed for issues nurses and nurse managers come across on a daily basis. Hildebrandt and Persily’s theory of community empowerment is a perfect example of how nursing management could overcome the challenges of relocating to a new unit.
