The purpose of this paper is present a synopsis of the article titled, The Effect of a Home-Based Exercise Intervention on Postnatal Depression and Fatigue: A Randomized Control Trial. This synopsis addresses the following items: purpose or objective, background, primary independent and dependent variables, main findings of the literature review, methods, statistical tests used and rationale for each, primary outcomes, results or findings, conclusions, and evidence rating using the SORT and GRADE evidence rating methods. Purpose, Objective(s)/Aim(s) Fatemeh Mohammadi, Jamileh Malakooti, Jalil Babapoor heiroddin, and Sakineh Mohammad-Alizadeh-Charandabi, 2014 stated, “this study aims to determine the effectiveness of home-based low-intensity stretching and breathing exercises on the reduction of 1 and 2 month post-partum depression (primary outcome) and fatigue (secondary outcome) scores.” Background and Variables The role of becoming a mother has many effects on a woman such as new roles, major responsibilities, money issues, increased unpaid workload, and harm to body image. These issues cause many women to suffer from postnatal fatigue and depression. These psychological issues can interfere with the mother’s ability to care for her new baby. The independent variable in this study is the home-based antenatal and postnatal exercise program. The dependent variable will be decreased depression and fatigue. Postpartum depression is one of the most common complications of childbearing with an estimated prevalence of 19.2% in the first three months after delivery (1). Depressive episodes (major and mild) may be experienced by approximately half of women during the first postpartum year (1). Characterized by depressed mood, loss of pleasure or interest in daily activities, feelings of worthlessness and guilt, irritability, sleep and eating disturbances (2), its etiology is multi-faceted and complex (3;4).
Million of Americans go through the blues and sad moments from clinical depression each year. Most patients with depression first seek treatment from a therapist or a primary care provider. However, exercise is a developmental behavior intervention that has displayed strong promise in alleviating symptoms of depression. The objective is examine whether physical activity is associated with depression in an average size population based on gender and age. A total of 310 participates ( 123 Male and 184 females and 3 other ) of UO students and other random volunteered to participate. All participants age ranged from 14 to 99, had to completed two self- report questionnaires. Besides answering question on their exercise habits during the past week, also took into account that all participants also completed CES-D questionnaire that determine their depression levels. In the surveyed there was a lower level of depression with more frequent weekly exercise give it a negative correlation. Also with the duration question, showed a significant negative correlation with the levels of depression. The efficiency of exercise in reducing depression cannot be determined because of the small size of participates and the lack of excellent quality research on population with adequate follow up
When a woman gives birth to a child, it can be one of the most joyous and exciting moments in her life, yet it can also be difficult and stressful. Often the time after birth is a filled with joy and happiness due to the arrival of a new baby. However, for some mothers the birth of a baby leads to some complicated feelings that are unexpected. Up to 85% of postpartum woman experience a mild depression called “baby blues” (Lowdermilk, Perry, Cashion, & Alden, 2012). Though baby blues is hard on these mothers, another form of depression, postpartum depression, can be even more debilitating to postpartum woman. Postpartum depression affects about 15% (Lowdermilk et al., 2012) of postpartum woman. This disorder is not only distressing to the mother but to the whole family unit. This is why it is important for the nurse to not only recognize the signs and symptoms of a mother with postpartum depression, but also hopefully provide preventative care for the benefit of everyone involved.
Postpartum psychiatric disorders, particularly depression, has become the most underdiagnosed complication in the United States. It can lead to increased costs of medical care, inappropriate medical care, child abuse and neglect, discontinuation of breastfeeding, and family dysfunction and adversely affects early brain development (Earls, 2010). Over 400,000 infants are born to mothers that are depressed. One of 7 new mothers (14.5%) experience depressive episodes that impair maternal role function. An episode of major or minor depression that occurs during pregnancy or the first 12 months after birth is called perinatal or postpartum depression (Wisner, Chambers & Sit, 2006). Mothers with postpartum depression experience feelings of extreme sadness, anxiety, and exhaustion that may make it difficult for them to complete daily care activities for themselves or for others (Postpartum Depression). The six stages of postpartum are denial, anger, bargaining, depression, acceptance and PTSD. These stages may affect any women regardless of age, race, ethnicity, or economic status. However only a physician can diagnose a woman with postpartum depression. It does not occur because of something a mother does or does not do, it’s a combination of physical and emotional factors. After childbirth, the levels of hormones in a woman’s body quickly drop; which may lead to chemical changes in her brain (Postpartum Depression). Unbalanced hormones may trigger mood swings.
Postpartum depression (PPD) affects at least 10-20% of new mothers. However, the true incidence may be much higher due to the fact that screening is not considered to be a standard practice, leaving PPD undetected and untreated in many women (Schaar & Hall, 2014). Postpartum depression not only negatively affects the mother; it also has a negative impact on the infant. For this reason, it is important for the health care providers caring for pregnant and postpartum mothers to screen them for risk factors associated with PPD, as well as educate them on ways to lessen their chances of getting PPD. It is also important for the health care providers to screen for PPD with a standardized tool like the Edinburgh Postnatal Depression Scale (EPDS), and to take action in treating it when it is suspected or diagnosed.

Although widely recognized and somewhat easy to diagnose, depression is an ignored and almost hidden, disease. In women, the statistics are especially grim for those who are pregnant or were recently pregnant. A great number of women suffer from postpartum depression; an illness which is often overlooked, misdiagnosed and untreated. Postpartum depression (PPD) has been defined as an emotional disorder that occurs in an estimated 10-15% of all women after childbirth (Liberto, 2010). Postpartum depression not only impacts the mother, but can cause long-term psychological challenges for the baby and create emotional turmoil for all family members.
Depression is a common problem during and after pregnancy; about thirteen percent of pregnant women and new mothers have depression (Women’s Health, par. 2). According to the National Institute of Mental Health, postpartum depression is defined as a mood disorder that can affect women after childbirth (National Institute of Mental Health, par. 2). Even though the mothers that suffer from postpartum depression often think it is their fault, postpartum depression can happen to any mother. This is because it is a disorder that is out of their control, it is common among many mothers and it is usually caused by a hormonal imbalance.
There are times when new mothers are misdiagnosed and are given a lesser form of this depression. This is called having the baby blues, which is when a new mother experience’s mood swings, and anxiety. These symptoms can last one or two weeks after giving birth, where postpartum depression can continue on for months at a time. Even with baby blues having such a shorter time frame even after 2 weeks’ these new mothers are not being diagnosed correctly. Mothers’ who were going through postpartum depression during the first three months of the child’s life were seen to be irritable and less engaged (Field 2011). Inadequate caregiving is also a major effect of postpartum depression (Field 2011). Mothers who have postpartum depression are less likely to continue breastfeeding if it becomes difficult (Field 2011). Infant’s with sleep problems are often put into unsafe sleep practices because of a mother’s postpartum depression (Field 2011). Infants of mothers with postpartum depression are less likely to attend well doctor’s appointments (Field 2011). Safety practices are often violated when a mother has postpartum depression (Field 2011). Some mothers have thoughts of harming their infants during postpartum depression (Field 2011). They also can have a fear of being alone with their child (Field 2011). Mothers with postpartum depression are also known to use harsher punishments (Field 2011).
Postpartum depression has some risk factor, and a study showed that a poor sleep during the third-trimester of pregnancy is a risk factor for postpartum depression, which is related with the age of the women (Wu 2014).Furthermore, Jomeen and Martin also found that during pregnancy the first three trimester are very important because according to the PSQI test poor sleep increase the symptoms of postpartum depression(2007).Also, another study showed that poor sleep habit has a stronger correlation with depressive symptoms (Goyal 2009). Additionally, women who had a history of previously depression are more likely to have postpartum depression (Kettunen 2014). This depression is a concern because women with postpartum depression show less early interactions with their infants than women who are not depressed (Field 2010). Mothers who have brought into this world a blessing have been preparing themselves for a big change in their life. They have been learning and educating themselves about how to be a good mother. Many mothers find it really hard to transition from being an independent woman without children to becoming a mother (Corrigan, Kwasky, & Groh, 2015). Adapting to motherhood can be a drastic change, and usually creates challenges that lead to feeling overwhelmed (Leger & Letourneau, 2015). When a newly mother begins experiencing stress or becomes emotional then there can be a possibility that they can encounter Postpartum Depression (Leger et al., 2015). Postpartum depression can be seen and experienced in many different ways, it all varies on every mother (Corrigan et al., 2015). Many different mental health issues can be seen including baby blues, postpartum depression, postpartum obsessive-compulsive disorder, and the most serious, postpartum psychosis (Tam & Leslie, 2001).
