Throughout this semester I have learned a lot about the importance of effective interprofessional care. I have observed two different sites that have provided a new insight into how provider care and interprofessional team care impacts the patients. The first site I attended was SLU head and neck oncology department. The second site I attended was Life Care Senior Center. These sites gave me examples of good interprofessional care as well as examples of where interprofessional care could seek to improve. At the SLU head and neck oncology department, the nurse practitioner and the ENT surgeon worked in cooperation with one another to provide the best health care services (IPEC, 1.5). Despite the amount of time the therapist spends with the patient, she stressed the importance of developing a trusting relationship with the patient (IPEC, 1.6). A similarity between both sets of health care providers at both sites is shared goals. All of the providers want their patient to receive the best possible care as well as to achieve their individual health goals. The difference among the health providers was the amount of time each professional spent with patients. The occupational therapist spent the longest amount of time with the patient followed by the Nurse Practitioner. Both ENT doctor and attending physician spent similar amount of time with each patient. Patients At SLU head and neck oncology, I interviewed a patient who has a new growth in her mandible. It is considered a tumor, but it is not cancerous. She describes the new growth as a knot in her jaw. This knot has caused her teeth to shift and pain in her mouth. The patient is new to the team, so she does not feel like an active member just yet. As for her care, she felt that the nurse practitioner and the ENT doctor were very nice and she appreciated the honesty of the ENT doctor. For this particular patient, the ENT doctor was very straightforward and blunt, which is what this patient needed. The difference between interdisciplinary teams and Interprofessional teams is that interdisciplinary team perform their work in a collaborative fashion. Each member of the team, while providing the group with the knowledge and skills of his or her disciplinary perspective, also strives to incorporate that perspective with those of others to create solutions to health care problems that transcend conventional, discipline-specific method, procedures, and techniques. Communication by far is essential to collaboration and interaction. However, Interprofessional teams has to do with learning together as a team. Interprofessional team collaboration for professional nurses is viewed as a method to improve the care and safety for patients. However, interprofessional team collaboration presents both advantages and challenges for nurses and other team members. One of the advantages is the coordination of care for the patient and the sharing of knowledge to improve the outcomes for the patient. Challenges for interprofessional team collaboration is: poor role-definition, miscommunication, conflict, lack of accountability for assignment of responsibilities and tasks (Reeves, 2012). Interprofessional care consist of multiple health care professionals from different professions working together on the same collaborative team to provide the best possible health care to the patient to ensure the best outcome for the patient. A interprofessional team consist of any health care professionals, for example physicians, pharmacist, social workers, nurses, and physical therapist. Each professional has there own defined role and their strengths are optimized to provide the best care to the patient. In order for the team to work team members must respect and value other members opinions and ideas about decisions for the patient, even if you might disagree with another members opinion. For the best possible chance of achieving the best outcome for the patient, everyone on the team must collaborate together to help the patient. The Pharmacist 's role has evolved over the past years. Before, pharmacist used to just dispense the medicine the physicians prescribed. Now, pharmacist are a integral part of the interprofessional team. Pharmacist now go on rounds with the team, talk to patients one on one to discuss how to manage there disease, and many other things. Even though a pharmacist does much more then just dispense medicine, many patients and some professionals still think that 's all they do. Zwarenstein et al (2007) designed and evaluated an intervention intended to improve interprofessional collaborative communication and patient-centred care. The intervention was aimed at the development of a hospital-based staff communication protocol designed to promote collaborative communication between healthcare professionals and ultimately, enhance patient-centred care within the team (Zwarentstein et al., 2007). The authors found that a substantial amount of interprofessional communication lacks three key core elements such as self-introduction, description of professional role, and solicitation of other professional perspectives (Zwarenstein et al, 2007). With these findings, the new protocol, based on the three identified core elements was designed to improve the overall culture of communication among healthcare providers.
Communication and conflict resolution are two very important aspects in order for a team to function effectively. This paper will explain different strategies for effective interprofessional teamwork and collaboration in health care delivery. Furthermore, evidence-based nursing interventions for systemic change to resolve conflict resolution will be discussed. Over the past ten weeks, I have developed a greater understanding of quality improvement initiatives and interprofessional care. I liked that the course focused both quality and interprofessional care. The linkage between these two topics is important, since everyone on the interprofessional care team is responsible for providing evidence-based quality care. Two topics I found interesting, that will benefit me professionally in the future, were the discussions regarding Quality Improvement Care Models and Interprofessional Collaboration. I work as home health nurse. Working in the home health setting requires excellent communication and collaboration between all staff members that provide care to patients. When I first began working home health, I feel I was on my own because I very rarely travel to the office and most of my time is spent driving and in the patients’ homes. I was quick to realize that I am never alone working home health. I can honestly say my fellow co-workers are always available to answer questions and to offer assistance when needed.
