The first opportunity is to make sure that there is excellent primary care. For instance, in the United States, there is a strong emphasize on specialized care and there is a lack of primary care, which is a “team-based” approach care. This team-based care would include a nurse, a physician, and a social worker. This is important because a team-based approach care will be able to better serve the need of older adults.
In addition, a second approach that would afford better health care for older adults in developed and developing countries is coordinated care. Unfortunately, for many older adults, they have to visit a physician, a social worker, or a nurse in order to receive the care they need. A suggestion that would better alleviate the
The challenge America now faces is the number of people reaching retirement will double in number by 2030, and the U.S. population will increase almost 20 percent ("Our Aging Nation," 2015). The goal is for the elderly to maintain and live with independence and dignity, as well as, provide a wide range of professional health and social service expertise, home care, and residential support and services that will be needed ("Our Aging Nation," 2015). Since the number of caregivers needed for this population will not be able to meet the demands other resolutions are considered necessary.
Current numbers show substantial growth from the eighties, and estimates suggest that the demand for long term care among the elderly will more than double in the next thirty years. (Feder, Komisar, and Niefeld) This growth will exacerbate concerns about balancing institutional and noninstitutional care, assuring quality of care, and most importantly adopting and sustaining financing mechanisms that equitably and adequately protect the elderly who need long-term care.
Taking care of the individuals that are getting older takes many different needs. Most of these needs cannot be given from the help of a family. This causes the need of having to put your love one into a home and causing for the worry of how they will be treated. It is important for the family and also the soon to be client to feel at home in their new environment. This has been an issue with the care being provided for each individual, which has lead to the need of making sure individuals have their own health care plan.
The primary care practice is essential to improve the care of our population, our current system is fragmented, but it does show potential for improvement. The Agency for Healthcare Research and Quality has listed some areas that will help improve our system. One is “the need for external infrastructure to help primary care practices develop quality improvement” this is done with support to the quality capacity (Agency For Healthcare Research and Quality, 2015). Quality care will include the coordination of care within the system, as well as understanding what needs the patient will have

Our elderly population is living longer than ever before and not all of them are entering into a nursing home. They are choosing to stay in their own home or their caregiver is choosing it for them. Some caregivers are choosing to move their ageing love one in the home with them. Whatever the case may be, there is an increased need for some type of home health as it applies to the elderly population. “Medicare will pay the full cost of professional help only if the physician
Health care has evolved and is continuously evolving. The management of care now involves different clinicians to better assess, diagnose and cure a patient. The clinicians evolved from a general practitioner to a team now comprised of Physician’s Assistant, Nurse, License Practical Nurse and Specialists. These health care professionals now compose a team of health care providers that are essential in a patient’s over all health care. The team-based approach is a delivery system that provides a patient an all-encompassing health care delivery system. “ By practicing in a team-based care model, physicians and other

The World Health Organisation (WHO) believe that it is important for health professionals to take a holistic approach towards the elderly patients, to see explore any other possible factors that will influence the reduction of their quality of life. (WHO, 2011).
Thus, a better healthcare policy for the older population may help to address their many health related problem and the financial burden. Implementing a populace health approach to health, which focuses on the interconnected conditions and aspects that influence the health of the older populations over their life course, recognize the systematic disparities in their forms of occurrence, and applies the knowledge to develop and implement policies to improve their health and to reduce health inequalities (Plouffe, 2003). With that, the Government can promote healthy aging that heightens the opportunities to improve and retain the seniors’ health, social wellbeing, independence and quality of
Most aging adults do not receive the necessary preventive services to help promote health and wellness during this stage of life. These preventive services include vaccinations and screenings. These important services help detect many diseases, delay their onset, or identify them early in their most treatable stages to ensure healthier, longer, and more productive lives for older adults (Prevention, 2011). Better people take care of themselves the longer they will live. People who balance: the right diet, making better choices and not acting sedentary has a better chance in living longer lives.
With the population of adults 65 and over steadily climbing, medical professionals and the healthcare system are struggling to keep up. One major issue concerning the healthcare system is that it is not set up properly provide care for these older adults. The physicians today, who regularly see patients, focus mainly has been; diagnose—usually in a 15-20 minute office visit, treat, and cure. Many of these senior citizens have multiple chronic illnesses or diseases, which require time to manage and treat. The problem is that they may not be able to explain what the real problem to a doctor in that time.
Teamwork is vital in healthcare. When all participants are engaged in a program, goals are successfully achieved. Being able to communicate and work collectively as a team requires an appreciation for each other’s area of practice. Every team member has an important role and being acknowledged provides a sense of responsibility and accountability. Essentially, inter-professional collaboration helps ensure that the patient is getting care that is not only accessible but also comprehensive. The plan of a patients’ care includes active participation by all health care professionals working interdependently in accordance to the patient’s preferences, values and beliefs. The health care team accomplishes the goal of meeting the patient’s medical needs by delivering evidence-based practice. To deliver quality care, the patient should always be involved.
Proper nutrition is another key to helping the older adult stay healthy. Many older people tend to say that they know what is involved in a healthy diet but do not feel that the can afford to eat healthy (Walsh, 2015). A third component of this plan is finding ways to help the older adult in finding ways to coping with the mental stressors of daily life. The focus of these components of this plan will be discussed more in the context of this paper and will include some suggestion that would further benefit those in the rural communities. Each of these areas that this plan would promote can be promoted by different health care providers. This paper would like to discuss the disciplines that will help to promote this plan, potential communications that would happen among these disciplines. This paper would also discuss how the incorporate health plan is a culturally based plan. This plan will empower the individuals who participate in the plan and assist them to maintain their
Nursing care delivery is defined as the way task allocation, responsibility, and authority are organized to achieve patient care. Tiedeman and Lookinland (2004) suggested that systems of nursing care delivery are a reflection of social values, management ideology, and economic considerations.
