Abstract: This report is an overview of the accident that occurred on American Airlines flight 1420 that occurred on a McDonnell Douglas MD-82 aircraft in Little Rock, Arkansas on June 1, 1999. The plane overran the runway during adverse weather that took place at the airport. The plane veered off the runway into some tubes, through a chain-linked fence and off into an embankment. This report will address weather information that took place that evening; the pilot and co-pilot decision making, and the structure of the airplane. It also addresses the controller actions during this period. It looks at the causes and findings in this report. It will also discuss recommendations for safety from the FAA, National Transportation Safety Board and the National Weather Service.
Introduction American Airlines flight 1420 was bound for Arkansas on June 1, 1999. It was the last flight of a three leg route. The plan crashed at 10:03 A.M into a field in Shanksville Pennsylvania at 580 mph. It was Boeing 757 that held 37 passengers and 7 crew members that all were killed from the crash. The flights path was originally suppose to fly from Newark, New Jersey to San Francisco, California, until four Muslim Hijackers took control of the plane and were headed toward Washington D.C (Avery). The causes of the crash are still being investigated but the likely causes include the plane touching down 2000 feet passed the runway threshold, the rapidly deteriorating weather, and a fatigued flight crew.

On September 19, 2008 at 11:53 pm eastern standard daylight time, a chartered Bombardier Learjet Model 60, N999LJ, owned by Inter Travel and Services, Inc., and operated by Global Exec Aviation, overran runway 11 during a RTO at Columbia Metropolitan Airport, Columbia, South Carolina. The aircraft crashed and burned after the crew was unable to stop the aircraft before departing the runway. The captain, the first officer, and two passengers were killed; two other passengers were seriously injured. The chartered passenger flight was headed to Van Nuys, California. The flight was ready for departure just prior to midnight. After analysis of the cockpit voice recorder (CVR) transcript, it was revealed that the flight crew received clearance instructions from the CAE ground controller to taxi from the northeast fixed-base operator’s (FBO) parking ramp to runway 11.
