Healthcare in the twenty-first century is transforming as swiftly as I type these words. Now, more than ever, patients, families, and teams of wellness providers are continuously striving towards comprehensive partnership development, to allow for the best possible patient outcomes. McGregor PACE (Program of All-Inclusive Care for the Elderly), is a managed care program in Cleveland, Ohio that has paved the way for over a decade, in the area of patient and family-centered care. The attached “Patient- and Family-Centered Care Organizational Tool” (Self-Assessment Tool, 2013) displays areas of facility strengths, as well as developmental areas within the organization. Federal legislation extended this new financing and allowed 10 additional organizations to open in other parts of the country (National PACE website, 2002, p. 2). The Robert Woods Johnson Foundation and the Retirement Research Foundation also provided funding to On Lok and the first replication sites in order to support the program. Eventually, in 1990, the first Programs of All-Inclusive Care for the Elderly (PACE) received Medicare and Medicaid waivers to operate (National PACE website, 2002, p. 2). In 1996, McGregor PACE opened its doors to those individuals 55 and over residing in Cuyahoga County. Today, there are over 300 McGregor PACE participants. The program site is located in Cleveland Heights, Ohio. Eligibility requirements also include state certification of the patient’s need for nursing home care and his/her ability to live safely in the community. McGregor PACE provides a multitude of services within the care continuum. Some of these services include adult day care, medical care, medications, transportation, hospital and nursing home care, social services and numerous medical specialties. Program participants reside in the community and receive services at the site or at a partnered facility, depending on their plan of care. McGregor PACE has an adult day care, as well as a medical/nursing clinic, therapy services, and specific areas designated for activities and meals.
