A widely acknowledged criterion for the technical quality of RP is the positive surgical margin (PSM) rate, and this rate has been shown to affect the risk of biochemical recurrence (BCR) after surgery. Although previous reports demonstrated predictors that may prolong operation time (OT) and raise the rate of positive surgical margin (PSM), there are no studies so far that evaluated the practical impact of adhesion of the Denonvilliers’ fascia due to prostatic bleeding by biopsy procedure on the PSM rate. CSI operates in conjunction with Dignity Health (which runs the hospital in Arroyo Grande) and the governing body or board (comprised of 49% [22-23] physicians who work within CSI and 51% shareholders from Dignity). The Medical Executive Committee (comprised of five board member physicians, among them the Medical Director and Anesthesia Director) answers to the board, and beneath the Medical Executive Committee is the medical staff. Medical Staff includes the Administrator: Director of Nursing, the Pathology Lab, and the Anesthesia Services. The structure of CSI follows Mintzberg’s five-sector model (Bolman & Deal, 2008) in that it is a top-down organization with a clear strategic apex consisting of the governing body of CSI. This administrative component funnels down to the Medical Executive Committee and ultimately the Director of Nursing who is directly in charge of the operating core, which consists of the nursing staff. The technostructure seems to be those specialists such as pathology and anesthesia services, while the support staff is comprised of front office and medical records personnel.

Surgical Technologists have an important role in the operation room (OR). There are different positions within the Surgical Technology field, including Scrub Surgical Technologist, Circulating Surgical Technologist, and Second Assisting Technologist. Scrub Surgical Technologists have a number of tasks, including prepping the patient for surgery, sterilizing the OR, gown and glove surgeons and assistants, and assists the surgeon and other surgical team members in a number of ways, such as passing instruments and dressing wounds. Circulating Surgical Technologists have a number of tasks as well, including checking patient’s charts, identifying patient and verifying the surgery that will be performed with consent forms, assisting anesthesia
As the ASC continued to grow in response to the increasing number of members, the procedures that were internalized not only increased, and were more complex. During that that time of expansion, I help strengthen the collaboration among ASC leadership and personnel to ensure that all surgical initiative is a success. In my role as perioperative educator, I spearheaded the development, implementation and evaluation of training and education via clinical simulation of procedures internalized in the ASC in 2015.

Organizational accountability and transparency has improved with the emergence of Hospital Inpatient Quality Reporting (IQR) programs and Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPs). This article will review the role of the nurse manager in creating a culture for quality care as well as the nurse for meeting organizational and patient expectations. Organizations like The Joint Commission (JC), Agency for Healthcare Research and Quality (AHRQ), Centers for Medicare and Medicaid Services (CMS), and The American Nurses Association (ANA) have been critical in establishing standards for quality. This paper will also report on the most recent hospital statistics and steps taken to improve HCAHP scores and reduce readmission rates at the University of Tennessee Medical Center in Knoxville (UTMCK). Statistics at UTMCK will also be compared to the Tennessee and National averages found on the Medicare website Hospital Compare. The aim of this paper is to explore if healthcare system initiatives are improving quality and enhancing patient outcomes.

Shared governance provides clinical providers, leadership, and an organization the opportunity to establish a mutual vision and a plan on how to get there (Brull, 2015). When shared governance is not applied to healthcare organizations many patient care staff feel as though their opinion does not matter. This model allows for staff to have higher job satisfaction, improved retention, and increased staff involvement (Sanford, 2016). Currently there is much success being found when there is collaboration and in healthcare with shared governance the stage for all of these is being set.

For this essay I will be reflecting on the influences on, how the role of the operating department practitioner (ODP) has developed within the multi-professional healthcare team. I will also be discussing in this text some of the historical, political, legal, social, and cultural influences of the ODP. And I will be reflecting on my personal experience working within the operating department for the first time as a student, using the Gibbs cycle (1988.).
