The RP stated that the resident is very lucid, alert, and was very uncomfortable about going in her diaper. The RP stated the resident is fond of sports and enjoys watching sports on the television. The RP stated the resident was enjoying the television when she was told she had to go to bed. When she complained a caregiver named Robert go into the resident's face and told her that she was going to bed. According to the RP the resident was intimidated by the caregiver. Although the resident no longer lives in the facility she fears that the caregiver will find and harm her. The reporting party stated on one occasion the resident was having respiratory problems and the facility contacted the RP. The RP questioned that staff and asked if the resident had a fever. The RP was told that the resident was agitated therefore the RP went to the facility to check on the resident. The RP asked the caregiver to take the resident's temperature using a digital thermometer. The RP stated on a visit she observed that her son's right eyebrow was swollen and escorted her son outside of the home. The RP stated the resident was issued a government cell phone, however the caregivers take it away from her and refuses to let her charge it. On 6/13/16 the police were called to the facility due to resident's inability to enter the facility. The North Hollywood Police Officers Winter (#38878) and Officer Radke (#39569) arrived to the home. The Officers removed the resident due to her being battered and concern for her safety. The reporting party (RP) stated a home visit was made on 4/8/16 for foster child Ronny Perdomo. During the visit the RP observed multiple bruising to the foster child's lower part of his spine, upper left shoulder, and on his back area. The RP stated she took pictures of the bruises. The foster mother Ana Ramos explanation for the bruising was the child is Anemic. The RP stated on 4/11/16 she consulted with her SCSW who advised her to take the child for a forensic exam. The reporting party (RP) stated minor Anthony Silver DOB: 6/19/99 resided in the facility until when he was given notice on 10/4/16. According to the RP, Anthony went AWOL from the facility on that date. While being transported into custody to Juvenile Hall he disclosed that staff members brought drugs and alcohol into the facility. The RP stated she spoke with the minor privately regarding his allegation and disclosed several staff members were involved in bring in pills, tobacco, alcohol, and meth into the facility. When the minor was asked which staff members, he replied he didn't know and could not remember. Consequently the RP continued to speak with the minor and he revealed the names of Gary Stanley, who no longer works in the facility.
The reporting party (RP) stated on 12/08/15 he arrived at the facility at approximately 5:30PM to pick up his 19 month old son Neil. Upon knocking on the door the RP inquired about his son and was told that he was napping. The RP stated he wanted to come into the home and obtain his son without waking him. According to the RP the licensee refused to let him into the home after requesting four different times to be allowed in the home. The reporting party (RP) received a voice message from resident Maria Hayes age 53 DOB: 10/26/63 Day Program, Solano Diversified Services, Inc. 1761 Broadway St. Suite 135, Vallejo, 94589 (707) 552-0300 on 4/11/17. The Day Program reported that Maria disclosed she was slapped in the face by her caregiver named Cathy (last name unknown); however, there is no Cathy associated with the facility. According to the RP the slap resulted in a nose bleed, no bruising according to the RP. There was however a pinkish, red mark. The reporting party (RP) stated on 12/17/16 foster mother Cheryl Lamb was mopping the floor while foster children Michael and Caleb were outside. Later the minors wanted to enter the home and were asked by the foster mother to put on sock due to the wet floor. Caleb complied and put on his sock and went inside the home, however, Michael was defiant and refuse to put on the socks. Michael was informed to put on the sock or to wait until the floor dried. Conversely Michael chose not to put on the sock and walked backwards on the floor, falling on his bottom. Michael stated he was pushed by the foster mother, thus he fell. The RP stated the eviction notice was not served to the resident but sent to the payee. Resident was not made aware of the eviction from the facility. The RP stated the facility is making false accusations against the resident in an effort to get rid of him. The resident is accused of using drugs which the RP stated is false. CCIB received a call from Reporting party (RP) reporting that on 6/19/17, resident Dale Graver (age 89, DOB: 03/21/28) was found by a staff member (unknown) unresponsive while outside in 108 degree heat. The resident resides in the memory care unit and it was reported that the resident was left outside and unsupervised for over 2 hours. The resident suffered severe heatstroke and is currently intubated in the hospital's ICU.
The nurse kept answering Patient B that they needed her bed because she was no longer telemetry monitored and the bed was required to monitor another cardiac patient. Patient B, thinking that she was the first patient in that room and Patient A is no longer telemetry monitored, responded by asking why can 't Patient A be move instead. The nurse abruptly replied, "Due to the possibility of Patient A having C-diff, we have to move you as a precautionary measure." This explanation occurred within the earshot of Patient A. Patient A became visibly upset and tearful, and vocalized that she believes her rights to privacy of her medical condition were violated. Patient B was moved to another room within the same unit. Subsequently, Patient A asked for another nurse to be assigned to her. However, to add fuel to the flame, in the process of providing bedside report to the replacement nurse, the outgoing nurse mentioned Patient A 's new cancer diagnosis in front of Patient A 's family. I am writing to inform you about the situation at the Mason Police Department in Illinois. The department has 90 full time officers varying in ages from 21 to 59. The officers serving at the department have varying tenures with some putting in more than ten years of service to the department. It has come to my attention that the department has come up with a policy where police officers with better performance receive increased compensation after some sort of performance review. It is open knowledge that the duties of police officers require very strong physical ability in most situations especially when it comes to apprehending suspected criminals. I feel that the city's pay-for-performance plan is discriminatory against elderly police officers whose physical ability will not match the young officers. To precise the department is culpable of furthering indirect discrimination because they have developed a strategy to categorize officers of certain age group and putting them at disadvantage by comparing them to officers of other age group. The department is indirectly presiding on a plot to create performance targets that older officers find particularly challenging resulting in younger officers posing performance level that is difficult to match by their relatively older counterparts (Quadagno, 2008).
