The reporting party (RP) stated a few months ago there was an 8 year old child named Jeremy (male) who was in the classroom with her 5 year old daughter. The RP stated the child was observed on several occasions being hostile and violent to younger children. The RP stated on one occasion while viewing the child on the facility camera tormenting and trying to suffocate a child named Victor with a plastic bag. When the incident was brought to the facility's attention the reply was he has mental issues and he probably did not have his morning medication. The 5 year old was left alone in the van and decided to go home. It was a while before the facility noticed the 5 year old was missing and began to search for her. The RP stated the child was found outside her home waiting for her parent and suffering from dehydration. Both the child and the responsible staff were both terminated. The RP stated at 6:30pm on 6/30/15 she arrived to pick up her daughter. As she walked into the building she was informed by the assistant director Noemi her daughter had head lice and could not return to the facility the following day. The RP looked through her daughter hair in the presence of the assistant director and notified the assistant director that her daughter did not have lice but a slight case of dandruff. The RP asked the assistant director to contact the director Denise and inform the RP's daughter did not have lice and should be allowed to attend school the next day. The RP asked the assistant director to address this issue in private and not in the presence of other parents. The RP stated her daughter was embarrassed regarding her having lice. The reporting party (RP) disclosed that Taylor Trusheim DOB: 4/3/99 was brought into Sutter/Yuba Behavioral Health for a Triage appointment to assess for possible services. Prior to the appointment Taylor's mother La Vonne Bettencourt received a call from a family member who brought up an incident which occurred when she was 4 years old. The family member mentioned an incident where the child was molested by the daycare provider's husband Bret Baker. Subsequently the mother discussed the information with Taylor who confirmed the molestation. The confirmation alarmed the mother who recalled the last day the child was at the daycare. RP stated law enforcement received a call from an anonymous caller stated an employee, Shemaiah Messengale grabbed the child Marley's arm which is fractured. Law enforcement contacted the child's mother to verify injuries. It was disclosed the child had suffered 4 fractures to her elbow and wrist. The RP stated law enforcement spoke with owner/licensee of the facility and the employee accused of causing the injury. They reported Marley sustained the injuries accidentally when she fell from slide. The RP stated the child was treated at Children's Hospital Los Angeles (CHLA) by Dr. Lindsey Andras who determined the injuries are consistent with a fall. The RP stated on a visit she observed that her son's right eyebrow was swollen and escorted her son outside of the home. The RP stated her son is non-verbal but understands and is able to shake his head yes and no. The RP asked her son if someone had hit him and he nodded yes. The RP stated she named off the names of the caregivers and when she came to a caregiver named Chips, her son identified the caregiver by nodding no. The RP stated the resident was enjoying the television when she was told she had to go to bed. When she complained a caregiver named Robert go into the resident's face and told her that she was going to bed. According to the RP the resident was intimidated by the caregiver. Although the resident no longer lives in the facility she fears that the caregiver will find and harm her. The reporting party stated on one occasion the resident was having respiratory problems and the facility contacted the RP. The RP questioned that staff and asked if the resident had a fever. The RP was told that the resident was agitated therefore the RP went to the facility to check on the resident. The RP asked the caregiver to take the resident's temperature using a digital thermometer. The reporting party is (RP) Ophelia Larose the grandmother of the foster child Aniyah Blake. The RP stated she and the biological mother had a visit with Aniyah on 6/19/15. The RP the foster father (Mark Cannon) did not bring food or diapers for the visit. The RP stated medical office has received 4 alleged abuse complaints from 4 different children. Each child stated Ms. Lisa was the perpetrator. The RP stated a 3 year old Hispanic girl was initially potty trained. The RP stated the child was eager and happy to attend the preschool however; the child began to soil herself and refused to back to the preschool. The RP when Dillion arrived home she observed that he had a bump on the side of his head and a split lip. The RP contacted the licensee and was told that Dillion fell out of a chair. The RP stated it was a highchair. According to the RP her 8 year old daughter Hannah Lopez witnessed the fall. The reporter stated on 11/19/15, the children had a forensic interview with Regan Smith, family nurse at Children’s Safe Center. The reporter stated after the interview, Ms. Smith made a recommendation that the children seek professional counseling for lack of supervision; Ms. Smith also recommended that Ms. Kiner is not capable of properly caring for the children. The reporting party (RP) stated minor Anthony Silver DOB: 6/19/99 resided in the facility until when he was given notice on 10/4/16. According to the RP, Anthony went AWOL from the facility on that date. While being transported into custody to Juvenile Hall he disclosed that staff members brought drugs and alcohol into the facility. The RP stated she spoke with the minor privately regarding his allegation and disclosed several staff members were involved in bring in pills, tobacco, alcohol, and meth into the facility. When the minor was asked which staff members, he replied he didn't know and could not remember. Consequently the RP continued to speak with the minor and he revealed the names of Gary Stanley, who no longer works in the facility.
The reporting party (RP) stated his daughter has had several falls resulting in injuries. According to the RP the falls were due to the teacher's lack of supervision. On 11/1/16 while walking down steps the child turned away from the RP 19 month child and loss contact causing the child to fall down the steps. Subsequently on 10/11/16 the child was bitten on 3 occasion on her left arm and elbow in one day. The reporter was contacted at 2:08pm at 09/01/2015 for additional information. The reporter said they were suspecting abuse, but didn’t specify. The reporter was asked was she suspecting molestation, and she replied that she didn’t want to say something that wasn’t true. The reporter never gave a clear answer to what she was alleging. The reporter said Jessica (teacher) has been very concerned about Harmony. Harmony transferred to Mooreville Elementary on 08/24/2015, and she has been arriving to school late (10:30am). It is unknown how many times the child has been tardy/late.
