The Sunrise Enabler Framework The Sunrise Enabler framework was developed by Madeleine M. Leininger, who was a pioneer in transcultural nursing. Leiningers’s framework encompasses the many domains that relate to an individual’s culture. Ultimately, the goal of this model is to serve as a guide in providing patients with care that is culturally congruent. The nurse or healthcare provider can accomplish this by assessing the different domains that relate to the model. This paper will focus on the domain of religion and spirituality within the Hindu religion. Since Hinduism is a very old religion, practices may vary among the different regions of India. The Sunrise Enabler model will serve as a framework to guide this cultural assessment.
The Informant and Setting
Sharmistha, the informant who was interviewed for this paper who has also granted permission for her name to be used, identifies herself as a Bengali from Calcutta, which is located in the eastern region of India. This interview took place in Sharmistha’s home, where the interviewer was invited to discuss the topic about death and dying. Before entering the home, shoes were to be removed and left at the door. The interviewer then followed Sharmistha past her living room into her dining room/kitchen. Sharmistha was very welcoming in offering her guest a cup of masala chai tea which was currently being prepared, the interviewer politely accepted.

“Health is influenced by culture and beliefs” (NRS-429V, 2011, p. 1). In order for the nurse to properly care for the patient, she must know and understand the patient’s culture. “Cultural care is a comprehensive model that includes the assessment of a client’s cultural needs, beliefs, and health care practices” (NRS-429V, 2011, p. 1). Knowing how to care for patients of different religious and spiritual faiths is essential to providing high-quality, patient-centered care. The author of this paper will research three lesser-known religions; Taoism, Sikhism and Shamanism. Through this paper, she will provide a brief background on each of the three religions and present information regarding spiritual perspectives on healing, critical components of healing and health care considerations associated with each religion.
In today’s world, where globalization and the internet has shrunken spaces and reduced distances, it is only natural that we come across people of different cultures living in harmony. As a nurse practitioner, one experiences several circumstances where the patients are from different cultural backgrounds, varied religious beliefs and also from the other part of the world. While a nursing practitioner has a challenge of treating every patient with utmost care and concern, there are times where our strengths and beliefs come in between our duty (Maier-Lorentz, 2008). A nurse practitioner might have to face situations where they might not have any control over, and still should show their best face. This is where their values, beliefs and behaviors might help them to achieve the impossible.
The Republic of Indonesia is a land full of many different cultures, ethnicities and beliefs. It consists of five large islands, and thousands of smaller islands, of which six thousand are inhabited (“Indonesia”, 2016). Indonesia is a little over nine thousand miles away from the United States, and with all that distance comes some distinct differences in lifestyle, beliefs, and social norms. As a nurse working in the healthcare field, it is our responsibility to be respectful of other people’s beliefs and values and to encourage diversity, even with our patients. Allowing our patients to continue their normal routines as much as possible and to feel comfortable decreases stress, which enables healing. It is hard to be respectful of another’s beliefs without first having an accurate data base of information about said person’s beliefs. This essay outlines detailed information about several different aspects of Indonesian culture including; family patterns, communication, space and time, nutritional patterns, health beliefs and practices, spiritual practices, and biological variations.
Leininger (1988) defined nursing as a transcultural phenomenon requiring knowledge of different cultures to provide care that is congruent with the clients’ life ways, social structure, and environmental context. This definition from the founder of transcultural care would support applying a cultural sensitive nursing care in the clinical setting, and highly linked to her definition of transcultural nursing as a legitimate and formal area of study focused on culturally based care beliefs, values, and practices to help cultures or subcultures maintain or regain their health (wellbeing) and face disabilities or death in culturally congruent and beneficial caring ways (Leininger, 1970, 1978, 1995). Moreover Leininger summarized the process of applying transcultural care as providing care that fits with cultural beliefs and life ways. From a professional perspective, it refers to the use of emic (local cultural knowledge and life ways) in meaningful and tailored ways that fit with etic (largely professional outsiders’ knowledge), in other words care should be individualized according to patient’s culture to promote holistic and effective nursing care.
Health in all cultures is an important aspect of life. A person’s cultural background, religion and/or beliefs, greatly influences a person’s health and their response to medical care (Spector, 2004). These diverse cultures guide decisions made in daily life; what food eaten, living arrangements made, medications taken and medical advice listened to. A nurse must be knowledgeable and respectful of these diverse cultures and understand their importance when providing care. The United States has always been the symbol of freedom of religion and health care today has needed to increase its knowledge in incorporating the many different religions/spiritual beliefs in order to provide a more holistic approach to care. As health care providers we should not look in validating our own practice in regards to religion or spirituality but to comprehend and learn to see the patterns of similarities and differences in order to provide holistic care to our patients. As religious and spiritual beliefs are never permanent and are constantly changing and/or influenced by government, thinkers, historical events, technology and the shifting values of cultures the study of religions/spirituality should be continuous for all health care providers. The Native American, Buddhism, and Sikhism have some similar traits and values imbedded in their practiced religions that resemble the Christian Faith and medical providers needs to be aware and able to accommodate them in order to provide holistic care.
Cultural competence can be defined as using the ability of one’s awareness, attitude, knowledge and skill to effectively interact with a patient’s many cultural differences. Madeline Leininger, a pioneer on transcultural nursing describes it this way; “a formal area of study and practice focused on comparative human-care differences and similarities of the beliefs, values and patterned lifeways of cultures to provide culturally congruent, meaningful, and beneficial health care to people” (Barker, 2009, p. 498). The nursing profession is known for being a caring and compassionate career path that is loving and accepting of all individuals. The traits of many of us are based on our cultural background and therefore it is important to be culturally aware of different traditions and preferences pertaining to cultures that we directly interact with. Both taking into consideration the patient’s culture and recognizing the impact of one’s own personal beliefs are competencies that a nurse should demonstrate and are listed under the first standard of practice according to the American Nurses Association (ANA, 2015).

The results were a reflection of my personal understanding how culture impacts holistic care and my continuous need to improve my care. I have strived to become non-judgmental. I work to not let my personal beliefs and values influence my nursing care on decisions. When I practice this philosophy it provides better patient outcomes and promotes an improved understanding of cultural differences.
