Traditional Indian Healthcare System in Delivering Primary Healthcare Services
Abstract: The current healthcare system across globe is heading in a way that, to maintain the health and vitality of human beings they have to address forces in both the natural and spiritual world. The traditional health care system and beliefs are grounded in cultural and religious practices of local communities. Historical circumstances, geography and demographics shape and inform the delivery of traditional healthcare systems. Modern healthcare systems are the result of a long evolution through trial and error and exchange of such information between diverse communities and regions. This process of exchange and gathering of information is still continued, today traditional medical practices are compelled to accommodate to the modern health care policies. The aim of this paper is to address on development of health care system to integrate the best of Indian traditional health care methods to meet the primary health care needs of contemporary society.
Body and mind are inseparable, balance between the two is necessary for healthy and meaningful life. Cultural diversity in the health care setting is increasing each year. Knowing how to care for patients of different religious and spiritual faiths is essential to providing high-quality, patient-centered care. The author of this paper will research three lesser-known religions; Taoism, Sikhism and Shamanism. Through this paper, she will provide a brief background on each of the three religions and present information regarding spiritual perspectives on healing, critical components of healing and health care considerations associated with each religion.
IHS is a very complex organization that serves the American Indian and Alaskan Native population. Effective health services for American Indians and Alaskan Natives had to integrate the philosophies of the tribes with those of the medical community. Because not all tribes signed treaties with the United States some people with Indian heritage were not eligible to participate with the federal government programs. Eligible people with Indian heritage were provided various services throughout the IHS programs; however, some IHS locations did not have the necessary equipment or facilities to provide comprehensive services.
Application of the Transcultural Analysis to an American Indian culture is accomplished through known cultural beliefs and practices. However, it is important to note that while each culture may have defining characteristics, family and individual practices may vary. To be an effective provider implementation must be tailored to individual needs. Starting with cultural awareness and building a care plan fit for the individual is ideal in the healthcare setting.
Two of the three models of health and healing discussed by Dossey (1999) were Era II Body/Mind Model of the 1950s and Era III Body/Mind/Spirit Model of the 1990s. Many traditional Native medicines and healing practices were discouraged with the advent of Western medicine, but now there is a movement to return to traditional ways (Zubek, 1994, p. 1924). Modern Western medicine treats the symptoms to cure a diseased state when the body is out of homeostasis. Native American healing traditions do this as well with herbs and plants suited to the purpose. These Native healing traditions also include sacred rituals, chants, and purification rites to help bring the spirit and mind of the afflicted back into balance. Biomedical physicians and traditional Native American healers are collaborating together to decipher how Native American treatments should proceed in the modern world and how to effectively introduce non-Natives to Native American healing practices. Hospitals and educational institutions including Harvard University, developed their own institution of alternative medicine. Doctor Lewis E. Mehl- Madrona’s 7 to 28 day intensive treatment program inspired by Native American healing traditions proved to have lasting effects on health improvement among those with chronic illness even 5 years after the start of the program. Native American traditional medicine and spiritual healing rituals go back for thousands of years, these traditions often focus on different variations of alternative medicine. This knowledge is passed on throughout generations, many of the tribes learn that by mixing natural plants such as herbs and roots they can make remedies with healing properties. It is believed that being healthy is when people reach a state of harmony not only spiritually, mentally but physically. After completing the “cultural competence checklist: personal reflection designed by T.D. Goode” for the first time, I was not surprised by my results. The results were a reflection of my personal understanding how culture impacts holistic care and my continuous need to improve my care. I have strived to become non-judgmental. I work to not let my personal beliefs and values influence my nursing care on decisions. When I practice this philosophy it provides better patient outcomes and promotes an improved understanding of cultural differences. This also enhances a safe and caring environment. “It is important for health care providers to recognize that care must be individualized and considerate of the cultural. Racial, and ethnic identity of the patients and their families” (Broome, 2006, p. 486) .There is much diversity in the beliefs and practices of the Indian culture and we should grant these individuals complete freedom to practice their personal cultural variations and the religion they choose.
The World Health Organisation defined health: “State of complete physical and social well-being and not merely the absence of disease and infirmity” (WHO, 1946 p.100). Many factors influence health such as family traits, behaviours, access to quality healthcare and environment (quality of air, bad/good water and housing conditions) (WHO, 1946). The Sunrise Enabler framework was developed by Madeleine M. Leininger, who was a pioneer in transcultural nursing. Leiningers’s framework encompasses the many domains that relate to an individual’s culture. Ultimately, the goal of this model is to serve as a guide in providing patients with care that is culturally congruent. The nurse or healthcare provider can accomplish this by assessing the different domains that relate to the model. This paper will focus on the domain of religion and spirituality within the Hindu religion. Since Hinduism is a very old religion, practices may vary among the different regions of India. The Sunrise Enabler model will serve as a framework to guide this cultural assessment.
