For the purpose of this discussion, I will explore three business principles discussed in Greg Fisher’s power point which are needed to maintain safe, quality, and patient-centered care. The three business principles selected include; decide what I do, decide who I serve, and managing employees. The discussion will be written through the perspective of a nurse manger of a home care agency. Lastly, I will discuss the importance of each business principles to myself and health care.
Nurse mangers in the home care setting, have a unique set of challenges that set them apart from several of their management counterparts. Nurse mangers must obtain a skill set and have recourses to help mange a workforce that provides a variety of services across a wide culturally diverse geographic area (Rosenfeld, Pyc, Rosati, & Marren, 2012).
The first business principle selected was to decide what I do. The business principle of determining what I do; requires that I broaden my vision, while narrowing my focus (Fisher, 2008). The home care company cliental is very culturally diverse. Patients range from pediatrics to older adult. Services range from wound care, respiratory care, cardiac monitoring, intravenous infusions, and numerous services. The diversity of care and services generates concerns regarding patient safety and quality of care due to the vast education required to care for the population. A high level of skilled nurses is needed to ensure understanding of patients needs and positive outcomes. This principle is to ensure that within the hospital, the patients dignity is respected, they are given their own independence, even if that means by supervision of staff and their safety is protected. It’s the staff’s responsibility to ensure that this need is met. For example, when it comes to giving the patients a bed bath, the staff must ensure that the patient’s privacy is protected by not exposing the patient and making sure no others can see by closing the curtains or by closing the door. The patients in the hospital should be encouraged to make their own decisions. Such as, a member of staff should take the menu around each of the patients in the morning and ask them what they would like to eat for that day. I experienced this first-hand during work placement as I was asked to go around the ward and ask each patient what they would like from the set menu. There are several different types of business ownership which are most commonly used in business’ and company’s today, these include; Co-operative which is a business owned by its employees, Partnership which is a business owned by between 2 and 20 people, Private limited which is a business owner by a small groups of people who have shares and a Public limited business is owned by private individuals by shares bought and sold on the stock market. In this era of healthcare reform, with the increasing complexity of care, declining reimbursement for services and increased accountability for patient outcomes, attention is focused on quality outcomes that yield a financial return on investment for the patient and the healthcare organization. While hospital leadership may embrace evidence-based practice, a mission critical environment requires making the business case for implementing evidence-based interventions by justifying the potential for cost savings. In order to demonstrate a return on investment for clinical interventions, Clinical Nurse Specialists’ (CNSs’) must learn business skills that allow them to justify a need for change in practice. Business skills include intervention planning that considers the resources involved in planning, implementing and evaluating interventions and the associated costs, known as the “business case.” The purpose of this paper is to describe the process of making a business case for developing CNS-led interventions. The business case framework was developed by CNS faculty to help CNS students develop these skills in making the business case for intervention development. This framework can be used as a tool for educating CNS students as well as for practicing CNSs.
