On March 39, 2017, at 1224hrs, VA police received a Duress Alarm for 1E127. Investigation revealed that the Veteran was upset over not being able to receive her Ulfima Five Tens Unit. The MSA Clerk informed her that she would have to attend a class on how to properly us the Tens Unit. The Veteran demanded she give her the Tens Unit, because she had use it in the past. When the MSA Clerk refused and told her she would schedule her an appointment, the Veteran got angry and called the Clerk a witch and walked off. VA Police made contact with the Veteran at Primary Care Desk B lobby. Where the veteran gave an omission and stated she had an appointment and because the clerk was not at her desk she missed it. Upon my arrival I observed a thin build white female walking out of the Ortho Clinic waiting area being followed by a black female RN (Hill), a white female social worker and Corporal Shanek. On Friday 09/16/2016 at 2300 hours, Security Officer Michelle Gallegos was badging visitors in the E.D. Lobby when the sister of a patient in E.D. # 32 lunged over the counter, knocking down a computer screen and a cup of water, then striking Officer Gallegos in her left arm. Security Officer (SO) Omar Alonso, was inside the E.D. Security Office, and responded immediately verbally instructing the visitor that police were going to be notified and that she needed to leave property immediately, and he also called Dispatch for immediate assistance. The female visitor was with 5 other females and her mother so they all followed Officer Alonso, outside of the E.D. Lobby. They were screaming and hollering that they were being treated disrespectfully by the badging officer who would not let them go back and visit her sister. Security Officers Ariel Weiland, Demetrio Flores, and Supervisor Steven Evans all responded by 2303 hours and helped to keep all 6 visitors outside while SO Alonso, conducted an investigation into the visitor’s allegations that they were being unfairly treated and were given erroneous information. He spoke to the nurse that Officer Gallegos spoke to when she needed information on the patient and permission for visitors to be given a pass to visit. I performed the first compliance verification on associate Imani Graham with nothing to report. I deemed your actions were blatant acts of insubordination and unbecoming of an officer. I will not tolerate this type of behavior and expect better from an officer of your caliber.
On February 7, 2017 at 1538hrs, I was dispatched to the information desk for possible weapons in a vehicle. Upon arrival I met with Veteran Raymond Blanchette - 2671. He had come in to facility to ask about his firearms that were in his motorhome. Blanchette state he lost his house and was living out of his RV. Further RV was his way to get back and forth to appointment. I decided to warn him about the weapons being in his RV. I based this decision on the fact he, had to use walker to get around, being Homeless and being an elderly man. I did inform him if brought the RV with weapons back on station we would charge him with a weapons violation.
A typical recurrent problematic situation for this veteran is that he is verbally aggressive with other individuals. The veteran displayed this aggressive behavior when the writer attempted to speak to him when he came back from his walk. The veteran was irate, spitting, screaming, stating he would be moving from VHN, and would no longer see me. The researched proposal by Manasi Deshpande entitled “A Call to Improve Campus Accessibility for the Mobility Impaired,” is a reasonable argument, by this student, to improve campus accessibility at the University of Texas. I believe this student supports her argument very well by using interviews and campus observations. The proposal emphasizes the challenges students with mobility impairments encounter while attending classes on campus. Ms. Deshpande includes her personal experiences, as well as the experiences of fellow students as supporting evidence. Frankly, it was a bit shocking to learn this student changed her major due to accessibility issues. This fact alone should be a strong argument for change. The proposal details the benefits for improving campus accessibility not only for students, but also faculty. Moreover, Ms. Deshpande adds to her argument by including statistics regarding the

On June 21, 2011 Spc Brown and I were on CQ duty. 1SG, the SDO and the Platoon Sergeants conducted a walkthrough of the unit’s footprint. I asked the patient to bear down (vagal maneuver) in an attempt to slow down her heart rate. The patient complied, and her heart rate remains tachycardic in the 130s to 140s. To my dismay, this patient did not have any as needed (prn) medication on board to help control her heart rate or any medication to help lower her blood pressure. In addition, this patient was being followed by a team of resident doctors during the day and by a cross cover resident at night. Upon completing my assessment and double-checking the patient's vitals signs, I called to notify the cross cover resident of the situation. After explaining the situation, cross cover resident had given me an order to administer 10 mg IVP hydralazine. After receiving this order, I questioned the resident about it and suggested a different medication such as metoprolol. To my dismay, the resident continued with his order and instructed me to administer the medication, and he hung up the phone before I could say another word. In questioning his order, I conferred with my fellow RNs, drug book, and the pharmacist, and they all agreed that hydralazine was not the correct medication for the patient.
