Sound; the majority of individuals that use the service at the hospice have some degree of hearing deficiency. It is helpful to ensure they have the hearing equipment on and working. When I speak to the group as a whole I will project my voice, talking clearly and slowly.
Visual; At the day care, only Mrs. P. has sensory loss to the progression that she finds it challenging to see pictures or written words clearly. When I initiate communication with her I will kneel down to her eye level and place my hand on her harm. Any information that she will need to take home will be in large print.
Electronics; speaking clock for those with sight sensory loss, also electronic devices for reading where it is possible to change the font to make it larger,
People communicate in order to establish and maintain relationships with others, to give and receive info and instructions, to understand and be understood, to share opinions, knowledge, feelings, and emotions, to give encouragement and show others they are valued. Communication is an essential tool a carer can use to meet the needs of people. It is the basic requirement of the job role to communicate with individuals and staff members on a daily basis. Communicating with other staff members ensures effective team working and continuity of care. It also ensures any health and safety issues and recognised and reported.
I work in a nursing home, is a place for people who don't need to be in a hospital but can't be cared for at home. mostly the people what have hearing impairments,visual impairments and or learning disabilities. In my work i need give attention for any person what have difficulty for Hearing, speaking, seeing, and having difficulty learning. i work at night for 12 hours, Most nursing homes have nursing aides and skilled nurses on hand 24 hours a day.
Confusion i.e. acute illness or neurological disease:- Observe their non-verbal communication, eliminate all distractions, allow them time to communicate, discuss only one item per sentence, do not challenge confused statements and use pictures
This essay will explain the different roles of effective communication in health and social care settings. The communication between colleagues service users and also why multi agency working is important in effective communication. It will include all the communication forms which are interpersonal interactions, communication and language needs and preferences. Effective communication when having conversation with the service users even work colleagues when having an effective conversation. No one will get confused and will be able to understand the conversation by explaining it in detail. Multi agency working is really important to effective communication because multi agency working is part of multi-disciplinary team. (Stretch and Whitehouse, 2010)
2.3 You need as much information as possible about the individual's communication needs to ensure that their wishes and needs are met. Talk to the individual, their family/carers, other professionals involved in their care. If aids are needed (hearing aids, picture cards, pen and paper) make sure that these are available. If language is a barrier maybe learning a few words of their language would help. If their needs change you need to adapt to the changes. Using the internet, reading books, Journals, reports and reviews are good way of obtaining information providing you know what you need.
A plan that could be used to clarify a misunderstanding would be if a service user could not understand ones line of questioning, pictures could be used to combat this if the misunderstanding was still apparent after using this method one could resort to writing down the question and asking it slowly and clearly so that all parties understand what is being asked or requested.
Communication is a process of passing or receiving messages, through individual or group of people. Lack of communication can minimize how people interaction with each other, sharing ideas, building a relation and understanding each other can only be achieve through communication. Also, in the care setting, employee, employer and the care user’s are able to deliver care provision and achieve their objection through communication. Communication enables understanding between people.

In a health and social care setting there are two different types of communication and those are verbal communication and non-verbal communication. Verbal communication is the way you talk to someone and non-verbal communication is your body language towards someone.

I observed an infant who is 1 month old, her name is Bianca. During my observations hours I could observe that the baby communicate with an adult not by words, but through expressions, gestures, and sounds. At the time when the baby wanted to eat, I saw that she started moving her head from one side to another and also she opened her mouth really big, at the time the adult told me that it was almost her time to eat, the adult waited until the time she had to eat, and the baby started to cry, it makes me think that she was communicated with the adult telling her that she was hungry by crying, while the adult was preparing her formula the baby put her hand in her mouth and she was still crying and crying, after that the adult gave her formula to the baby and she ate the milk so quickly, and she slept.
Report, care plans, letter etc. these can be a way of communicating with someone who does not speak. Picture cards or sign language can be used for people who have a hearing impairment.

There is a direct correlation with communication, improving a patient's well being, and quality of care. Adequate communication among physicians and their patients is an actively growing research topic. These recommendations include the patient-physician relationship, how physicians utilize medical information, how physicians deal with patient emotions, physician self-management, and educational conferences designed to sharpen communication.
