Summary Introduction

To determine:

Describe the critical thinking skill that Tonya is demonstrating?

Concept introduction:

Mr.L is stabilized after his medical treatment since he had a pulmonary embolus. He is taking anticoagulants medication which likely reduces more clot formation. Mr.L underwent a surgery recently and still, he is continuing anticoagulant which will increase the bleeding from his wound. Tonya believes that the patient is at risk for bleeding from his surgical wound by increasing her assessment of the wound and teaching Mr.L to avoid placing stress on the wound.

Explanation of Solution

Since Mr.L had pulmonary embolus and continuing anticoagulant medication for it. Anticoagulant medication makes the blood thin and preventing it from clot formation. Mr.L underwent surgery and bleeding from his wound will become severe if he continues the anticoagulant medication. Any surgical wounds will not be healed easily in case the patient takes anticoagulant medication which prevents the platelet to aggregate near the wound site and helps in clot formation to close the wounds.

Assessing these factors Tonya applies a clinical judgment, the interpretation of a patient’s needs, concerns or health problems and the decision to take action or not.

Conclusion

Tonya application of clinical judgment is correct in the case of Mr.L since his surgical wounds will not heal and intensify the bleeding from the wound if he continues his anticoagulation medication. Until the wound heals Mr.L have to stop taking anticoagulation medication.
