Case 1

Why did the patient's PCWP, Cl, SVR, and urine output all improve after the administration of nitroprusside?

Summary Introduction

To review:

The reason for the improvement in the pulmonary capillary wedge pressure (PCWP), cardiac index (CI), systemic vascular resistance (SVR), and urine output after nitroprusside administration.

Introduction:

PCWP is the pressure calculated by fixing the pulmonary catheter with an inflated balloon into the small pulmonary arterial division. CI is the hemodynamic parameter, which relates cardiac output from the left ventricle in a minute to the body surface area. SVR is the resistance put by systemic circulation.

Explanation

In the given case, the adverse ripple effects of an increased afterload were observed on a patient’s parameters. The hemodynamic profile showed a very high systemic vascular resistance (SVR) and PCWP (pulmonary capillary wedge pressure), but a low CI (cardiac index). Nitroprusside was administered to the patient.

Nitroprusside is a medication that lowers blood pressure. It acts rapidly by dilating the blood vessels, on account of its breakdown in blood circulation to liberate nitric oxide (NO). It does so by attaching to the oxyhemoglobin to release nitric oxide, methemoglobin, and cyanide.

The NO stimulates the guanylate cyclase in the vascular smooth muscles and enhances the formation of cyclic guanosine monophosphate (cGMP)...
