Introduction:

The PaCO2/HCO3/pH nomogram is very useful to determine the acid-base status of the blood. It is also used to confirm whether the patient is facing imbalance in the acid and base in the blood or there is a deficiency of ions in the body.

Explanation

The patient was having low PaO2 (arterial pressure of oxygen) as the head of the patient was not placed properly. The improper position of the head led to inadequate ventilation of the lungs. This further caused an increase in the PaCO2 (arterial pressure of carbon dioxide), which led to a rapid fall in the pH (potential of hydrogen). The decrease in the PaO2 causes a further decrease in the pH.
