- Pretend that you have a mobility impairment (i.e., you haveto use crutches or a walker, cane, or wheelchair), and attempt to perform your usual daily activities, ideally including visit-ing a public place, such as a school or mall. How did you feel about the restriction of your movement? How can nurses bestassist patients who are coping with these restrictions? Howdid the public respond to your impairment, and what effectsmight such responses have on individuals with mobilityimpairments? Are public spaces adequately adapted to meetthe needs of those with mobility impairments? What measuresare needed to address any deficiencies? Did you identify anysafety needs? What advice would you give someone in terms of pain medication when they are discharged from hospital?Watch this short video (http://openstaxcollege.org/l/facialnerve) to see an examination of the facial nerve using some simple tests. The facial nerve controls the muscles of facial expression. Severe deficits will be obvious in watching someone use those muscles for normal control. One side of the face might not move like the other side. But directed tests, especially for contraction against resistance, require a formal testing of the muscles. The muscles of the upper and lower face need to be tested. The strength test in this video involves the patient squeezing her eyes shut and the examiner trying to pry her eyes open. Why does the examiner ask her to try a second time?
- Despite the widespread use of opioids to treat chronic musculoskeletal pain there is little quality data on their effectiveness, especially over the long term. The current study was designed to compare the effectiveness of opioid medication vs nonopioid medication over 12 months for patients with moderate to severe chronic back pain or hip or knee osteoarthritis pain. Outcomes were pain-related function and pain intensity. The results showed that treatment with opioids was not superior to treatment with non-opioid medications for improving function and in fact pain intensity was significantly lower with non-opioid treatment. In addition, adverse medication-related symptoms were significantly more common in the opioid group. The authors conclude that commencing opioid therapy for moderate to severe musculoskeletal pain is not supported. Question: Is it true, as it was once suggested, that people on prescribed opioids are less likely to become addicted to them?A patient in an intensive care burn unit for 1 week is inpain much of the time and has his face and both armsheavily bandaged. Encouraging regular use of analgesicsb. Applying a moist heating pad to the area at prescribedintervalsc. Reviewing the pain experience with the patientd. Ambulating the patient after administering medication
- describe the effects of pain gate theory in relation to manual therapyPain produced by muscle spasm results from :-a- mechanical stimulation of pain receptor by muscle spasmb- decreased release of lactic acid from the spastic muscle fibersc- release of compounds from the spastic muscle which increase the threshold for stimulation of pain receptorsd- decreased oxygen supply to the muscleA 67-year-old male patient arrives in the emergency department complaining of progressively worsening bouts of right leg weakness. His wife notes that he is confused and often has trouble forming words at these times, which typically last about a day. The right leg weakness improves only slightly from instance to instance, leaving him weaker each time. Because the symptoms are transient, he has put off seeing a physician until now. His wife is worried that he might be having a stroke. He reports having smoked a pack of cigarettes each day for 45 years. • What surgical procedure will be scheduled? • What tests might the ED physician order to arrive at a diagnosis? • What are the possible diagnoses, and which is most likely?
