On 06/01/2016 at 8:55 AM I along with Cpl. Pries, R. was escorting the above named inmate from the BHU Interview Room back to his assigned housing, BHU-13. Once we entered Section II of the BHU the inmate attempted to kick and head-butt Cpl. Pries and me. Because of this we had to place the inmate on the ground in order to stop his behavior and to gain control of him. Once the inmate was under control he was taken back to assigned cell. At approx. 9:15 AM the inmate was seen by the SMH nurse, RN Henkleman, P.
The reporting party (RP) stated resident Arev Dabbagh DOB: 12/27/00 disclosed on 4/15/16 she inappropriately touched by a male peer. According to the Arev on 4/14/16 a peer Ibraheem Mamoori DOB: 9/2/98) placed his hand on her shoulder and continued downwards towards her breast. The peer touched her breast over he clothing. The resident was distraught while she recounted the incident. Arev alluded to their being other incidents with the peer and other unknown males. Arev didn't disclose details or clarification. Arev was placed voluntarily in the facility by her mother and will continue her placement for a few more weeks. The peer is currently placed on a 5150 hold and will not have access to Arev in the future.

On 08/20/2017 at 1002 hours FTO Wheeler #4361 and I responded to St. Luke’s Hospital emergency room entrance, on 3555 Cesar Chavez St., regarding a women in a hospital gown trying to break the caller’s vehicle window with a brick. Officer Wheeler and I were in full police uniform driving a marked patrol unit when we responded.
On Wednesday 09/21/2016 at approximately 2056 hours, Security Officers Lourdes Garay and Supervisor Steven Evans were dispatched to ICU room #4112 for a (53B) Disorderly Baker Act Patient in Medical Unit. Upon arrival, Officers saw Nurse Cassandre Jermaine and Charge Nurse Cristina Sisneski attempting to calm down an irate Baker Act patient. The patient Adam Bargar (DOB: 02/05/77, FIN #86198457) was upset about not being able to make a phone call, he then ripped his IV out and attempting to leave the unit. I explained to him what a Baker Act patient is allowed to do and what limitations are obligatory. At approximately 2100 hours, I escorted Inmate Hernandez, Joesph T403455 from A05001 to Durango Jail Medical Clinic to be evaluated for injuries. Inmate Hernandez was evaluated by RN Anthony 1297H at 2145 hours. Inmate Hernandez was offered Administrative Restrictive Housing but refused and signed a refusal form.
On 08/14/2015 I was assigned as the Tower 13 Floor officer at the Lower Buckeye Jail (address located above) at approximately 1537 hours inmate Downd, Michael MCSO booking number T197703 came out to one core and claimed he was assaulted in Tower 13 A Pod cell 4. I reviewed video in the level one supervisor’s officer at about 1540 hours. I observed on camera 1308 T13 A pod PTZ multiple inmates standing in front of and around cell 4. At 1516 hours I observed Inmate Ludwig, Austin (S5) MCSO booking number T188789 enter the cell first as (S4) Inmate Dahl Ujacob MCSO booking number T191055 and (S6) Inmate Buchan, Devon MCSO booking number T183777 standing as lookouts in front of the cell door.
On 10/11/15 at approximately 1944 ICS was activated to an offender OID 236740 who was bleeding and Medical was requested to respond. Upon arrival offender was sitting on the chair in no apparent distress. Offender had dry blood on her hands and on the head. Offender was brought to the clinic and was assessed treated per protocol. The reporting party (RP) arrived at the facility on 6/9/16 at 7:40AM in response to male resident Douglass Gass DOB: 7/3/42 "911" call. The resident had fallen at approximately 11:00PM the night before. Consequently the resident was non-ambulatory and sustained minor injuries. The resident was treated and transported to the VA Hospital in Westwood. The RP disclosed that the resident was the "911" caller who stated he had been on the floor the entire night and no caregiver responded to his aid. During the conclusion of the call a caregiver appeared and was of the ongoing incident. The RP state several concerning conditions were observed. The facility was in violation of several Fire Code items. The resident and his roommate had no operable signal system.
On Thursday 03/10/2016 at approximately 1937 hours, Security Officer Omar Alonso reported to room # 503 Bed 1 for a (44P) Enforcement Escort Baker Act Patient Within Facility at 1942 hours. The patient, Rachel Gardner (DOB: 12/12/1985; FIN# 85335948) was uncooperative and not willing to go voluntarily to the Lakeside Psychiatric Facility transport van. She stated that she was missing her clothing, shoes, and purse. Security Officer Alonso met with Charge Nurse Tonya Smith who could not find any information in the patient's chart about personal belongings logged in. Nurse Smith contacted the Emergency Department for assistance while Security officer Alonso called Orange County Sheriff's for assistance with the patient who was also stating that
PER REPORTER: Perryman is in the custody of Forrest County DHS. Perryman made allegations that the staff member (Bruce) grabbed his arm and pushed him and said "this is what you want me to do". This incident happened last night around 7:30 p.m. Per reporter, the patient said they started arguing The patient was breaking rules and was redirected for walking out of his assigned area in the community. The patient approached a girl which isn't allowed. The nurse did an assessment. There is a red mark and scratch on his left upper arm. There are some pictures being taken. At approximately 1715 a code blue was called on inmate Graves, Sammie 17006700 in Cell G-8. Inmate was complaining of feeling faint and elevated blood pressure. All available alternates responded and medical Nurse Green, Nurse Carnley and nurse Peavy and Nurse Christiansen responded to G block. After assessing the situation inmate Graves was cleared and he is being housed in F-8 on 15-minute medical observation.
On 6/13/116 at approximately 1811 ICS was activated in RVST unit for offender Mcfarland OID 179096 who fell on the floor in the shower room and medical was requested to respond. Upon arrival on the scene offender was found on the floor naked, hyperventilating, shaking, cold, unresponsive. She was alert and conciuos. Intial assessement was completed on the scene and offender was lifted from from the floor by staff and was placed in a wheelchair. As soon as she was placed in the wheelchair, she started making verbal statements and answering simple questions. Offender was assisted by medical staff to get dressed and was wheeled to the clinic for further evaluation. offender was assessed in the clinic and had no signs of improvement. On call
Inmate Watson has a history of advanced glaucoma on latanoprost and timolol both eyes and presents for intraocular pressure check. Inmate states that the timolol causes him "sweating and lightheadedness." So he is not using the timolol.
1) I would utilize my Techno Beams #701 thru #704 with some yellow color at almost its full intensity and tilt them all the way up as I also load them with a color chase effect so that they can have this alternation pattern with each other in which all of these fixtures would open and close their dimmers as they alternate with each other, I would also play with the speed of this effect to get it as close as possible with the drummer since I think it would be a great idea if these fixtures fire up with the beat of the song as the drummer plays kicks his kick drum.
