As per OMNI, he was initially diagnosed with lumbar spine sprain/strain and gastritis secondary to non-steroidal anti-inflammatory drugs. Patient is status post left-sided L4-5 discectomy on 03/11/2015.

DOI: 5/13/2014. The patient is a 29-year old female manufacturing lead who sustained a work-related injury to her back when she was lifting two ostellation trays which weigh about 30 pounds.

DOI: 7/26/2012. The patient is a 44-year old female pickup associate who sustained a work-related injury to her hip while moving a barbecue grill. As per OMNI entry, she was initially diagnosed with lumbar strain and sacroiliac joint pain.

DOI: 7/14/2015. Patient is a 32-year-old male carpenter who sustained a work-related injury when he slipped with tanks falling on him as he was moving cart with cutting torch and tanks uphill. As per Omni, the patient has herniation at L4-L5 level.

The patient is a 70-year-old gentleman who presented with left hip pain of 3 weeks duration which has been worsening over the past 5 days 10/10 nonradiating pressure-like pain is worse on exertion and improved with rest is persistent. The patient's medical history is significant for gout, hypertension, diabetes, coronary disease, hypercholesterolemia, arthritis and nephrolithiasis. The patient had a right total knee replacement, 3 caths (the last one in 2003), a right rotator cuff repair, a right thumb injury and has had 6 previous hospital admissions for the right total knee replacement. OI: 11/22/2009. Patient is a 46-year-old female certified nursing assistant who sustained a work-related injury to her low back while helping move/lift a patient. Per OMNI, the patient is diagnosed with status post lumbar laminectomy surgery on 07/26/10, lumbar degenerative disc disease, and lumbar radiculopathy. She was deemed to have reached permanent and stationary status by PQME Dr. Well last 12/01/11.

DOI: 8/1/2014. Patient is a 46-year-old female relationships manager who incurred injury to her back from a horse at a camp site. As per OMNI, she was initially diagnosed with lumbar strain, thoracic strain, contusion at the low back and muscle spasm.
