1. On the morning of 7/28/2015 I received a transforminal epidural nerve root block with Dr. Cindy Zhang at the Pain and Arthritis Office in Woodbridge, VA. I received the shot along my spine in the L4, L5 and S1 region (which are the nerves in my lower right foot/ankle/leg. I receive these shots for chronic pain in my right leg. I have good strength in my right lower leg and it does not hinder me in driving. I have since called area facilities who perform these shots ….. (I am learning about them, unfortunately now after this incident, due to Dr. Zhang’s lack of communication with me as a patient the risks that are involved and what to expect after procedure) and have found that doctors have you wait in a recovery room (post-procedure)…… and have someone drive you after these shots! Dr. Zhang did not have me wait, check me as well as my leg to make sure everything was fine. She does not have you sign any papers and she does not have a post-procedure protocol! The approx. time of me entering her exam room was 11:30am and the accident happened around 12:00 noon. This sheds light on how quickly Dr. Zhang performed the shots and sent me on my way. 2. At the scene of the accident, my husband and son arrived. They were very concerned about my emotional state as well as sitting in the direct heat and sun. It was one of the hottest days that we have had and after the accident, I was sitting on a crate outside 7-11 by my car, and the extreme heat and the sun was
This case is extremely relevant to what is known as the four D’s of negligence; duty, dereliction, direct cause and damages. Duty is when a doctor and a patient have formed a relationship and said doctor has taken on the responsibility of taking care of the patient. Dereliction or failure to perform a duty, there must be some kind of proof that the doctor somehow neglected the doctor neglected the patient. Direct cause, there must be some kind of proof that what happened to the patient was a direct cause of how the doctor conducted himself or his failure to act which resulted in injury. Damages a patient must prove that harm was incurred by the direct result of the physicians actions.
Aldina, thank you for the effort in describing the incident that occurred at Bright Road, and the implications this had for Bright Road and Dr. Fredericks. There were a few good points that you made when trying to support your report, first, you were able to clarify that Mr. Davis had made changes to his consent form prior to the treatment. Then, that Dr. Fredericks agreed that he relied on what he viewed as the patient’s verbal consent to proceed with the procedure, and that he probably did not look at the written consent before the surgery. You were able to identify that this procedure caused long-term harm to Mr. Davis, because he would have to use additional medication that can put him in danger, as well as the potential problem with the

October 16th 2013 I was at work performing my normal duties, when as I lifted on a box to put it on a self above my head, I felt and heard a tearing/popping sound coming from my left shoulder. I was in serious pain after that. My employer had me feel out an accident report, and then sent me to their doctor. After being examined by the doctor, I was told that I just strained my left shoulder and put me on a temporary restriction, which limited the amount of weight I could lift. I was also told to come back for physical therapy once a week. After 5 weeks of physical therapy and with the pain almost unbearable, I decided to go to my own doctor. My doctor examined me and immediately scheduled an MRI. The MRI showed that I had a tear down the

Can you image having something lounged in your knee causing unbearable pain after surgery. In 2010 a patient went to a local hospital in Woodland Hills California, to have a knee replacement surgery done, and the operating surgeon left a foreign object inside the knee after performing surgery.(CDOPH). (Jones 2012). It was found that their was a unidentified nurse working in the in the surgical unit who failed to do a correct count on the sponges before surgery and after. Further more she forged a technician signature on the operating room record who was suppose to do the count himself, but failed to do his job. The doctor himself did not follow the correct procedure, and stuffed a 4 by 4 saturated sponge with anesthetic inside of the
This is simply not true what so ever. I am in severe pain and stated that I want to get to the bottom of pain not just continue to mask the problem with a pain pill. I did fill the pain pill prescriptions that had been prescribed to me. I am in severe pain and I know they wouldn't want to just suffer with out any pain medication while waiting to be diagnosed. I feel that she (Sandi) violated my HIPAA rights as well as slandered me. I have already been struggling with this pain and trying to find someone willing to help me figure out what is wrong with me. She humiliated me in front of everyone and just as hurtful it wasn't true. I just need someone who actually cares enough to help me figure out what is wrong with me. Nashville Metro General Hospital claimed they would but they was focus on my heart problem. My heart isn't hurting me right now. It's this other unknown problem that I need medical help trying to figure it out this is why I went there in the first place. When I went to Nashville Metro General on 16th of August and saw Dr. Soleye and Dr. John

He told my parents that for them, he would give me an extra injection. It was against the law to give me the extra dosage, and he could have lost his medical license if he was caught giving me the injection. Yet, for some reason, he was willing to do it for me, my parents, and my family. The doctor would perform the procedure as long as my parents would agree not to sue him. My parents were desperate and realized this was my only hope. They agreed with the procedure and continued on. I was injected with a third dose of the sealant for my
