MICU 15A was dispatched to 30 West Ave, @ Genesis Healthcare, Wayne Center in the Wayne Business District in Radnor Township, for an ALS Emergency, Assault Victim. The weather conditions were cloudy, cool and dry. MICU 15A responded with care per protocol to the stated location with EMT Straub driving. On arrival, the EMS Crew proceeded to the nurse's station, and the individuals there did not know anything about an incident, there was a female standing in one of the hallways who advised that there was a person sitting over where she was. The EMS crew proceeded to that area and found a 43-year old female who was a registered nurse and was conscious and alert x4 and was sitting on a chair, the EMS crew asked her what was happening, "she replied that she was in a patient room and stated that the patient had an IV in her arm and became disturbed and picked up a pocketbook and then assaulted her by the swing it and striking her in the right frontal region of the head", the nurse then exited the room and went and sat down in the hallway, where the EMS crew found her. …show more content…

On 3/13/16 SO EMT Perez was notified that an engine had come on campus. SO EMT Cunningham responded along with SO EMT Perez and caught up with the engine by lakeside commons. The driver of the engine stated that he was responding for a lift assist to 3144/3146 apt 220 but that he wasn’t sure which one he was going too. SO EMT Cunningham and SO EMT Perez decided to respond to FR-220 first being that they were the engine came from gate 2 and they were closest to that building. As I was completing my initial investigation I was returning items (driver’s licenses, registration, and insurance information) to the drivers. I approached Mitchell who was sitting in her vehicle. She appeared slumped over. I knocked on the window several times. There was no reply. I opened Mitchell’s vehicle door. She appeared unconscious. I shook her by the shoulder. She did not respond. I called for Goodwill Ambulance, who had just left with Feeney to return because I had an unresponsive person. I pulled Mitchell out of her vehicle and laid her on the street. On Friday, September 30, 2016 at approximately 0536 hours, a Black Male Stab Wounds Victim walked into the Emergency Department lobby with a severe Stab wounds laceration to the lower leg and a human bite wound. Security staff was contacted by E.D. triage Nurse Tony Rusiano and escorted the patient to ED room#42. Security conferred with E.D. Charge Nurse, Johnathan Bacal and an E.D. On 10/21/2015 SO EMT Perez was dispatched to HS-318 regaurding a general assistance call. SO EMT Perez knocked and announced his presnece at the door and was verbally greeted in by the resident, a Mrs. Netha Hopson. Mrs. Netha Hopson stated that she needed help retrieving her purse because her motorized chair was caught on it and she could not get it untangled on her own. SO EMT Perez managed to untangle the purse and retrieve it for Mrs. Netha Hopson. After insuring that Mrs. Netha Hopson needed nothing else SO EMT Perez cleared the scene without further incident.
On 10/4/2015 S/O EMT Perez was dispatched to MG-T15. S/O EMT Perez knocked and announced his presence at the door and was greeted by the resident, a Mrs. Maxine Montgomery. Mrs. Maxine Montgomery stated that she needed some help re ajusting her sling and was unable to do it herself. S/O EMT Perez adjusted her sling and asked if Mrs. Maxine Montgomery needed any asistance with anything else and or needed any aditional care. Mrs Maxine Montgomery stated that she was fine and needed no adititional care and escorted S/O EMT Perez out. S/O EMT Perez cleared the scene without further incident.
During the argument David was racially abused by the patient. After the incident David was moved to another ward whilst the other patient remained on the ward. That night, whilst David was on the other ward, he lashed out and hit a nurse. Following this he was restrained by five nurses and a struggle developed. The correct procedures for restraining a patient were not followed; subsequently, David collapsed and died (NSCSHA, 2003).

On Friday 09/16/2016 at 2300 hours, Security Officer Michelle Gallegos was badging visitors in the E.D. Lobby when the sister of a patient in E.D. # 32 lunged over the counter, knocking down a computer screen and a cup of water, then striking Officer Gallegos in her left arm. Security Officer (SO) Omar Alonso, was inside the E.D. Security Office, and responded immediately verbally instructing the visitor that police were going to be notified and that she needed to leave property immediately, and he also called Dispatch for immediate assistance. The female visitor was with 5 other females and her mother so they all followed Officer Alonso, outside of the E.D. Lobby. They were screaming and hollering that they were being treated disrespectfully by the badging officer who would not let them go back and visit her sister. Security Officers Ariel Weiland, Demetrio Flores, and Supervisor Steven Evans all responded by 2303 hours and helped to keep all 6 visitors outside while SO Alonso, conducted an investigation into the visitor’s allegations that they were being unfairly treated and were given erroneous information. During the home health observation day, there were several opportunities to observe a variety of patients with varying levels of functioning ability, different illnesses, and different needs and levels of interaction with the nurse. The first patient seen was a seventy-three year old Caucasian female with an ulcer on her right heel. Several weeks prior, she had scratched her left leg and she also had several small wounds on her left leg. The orders were to clean and redress the ulcer. She has a history of end stage renal disease, pneumonia, weakness, diabetes, dialysis, and right hip fracture. Upon entering the home, the patient was found to be sitting in a wheel chair in the living room watching television with her husband close by her side.
